Licensed Practical Nurse/Licensed Vocational Nurse Part Time Position Range: $27 - $31 per hour This is a fantastic role to continue your nursing career path and work within a setting that offers variety and rewarding challenges. You will have high-impact work by providing nursing services, training, and supports to address the medical or psychiatric needs of individuals with a brain injury, intellectual and developmental disability, a substance use disorder, and/or has an emotional or behavioral disability. Monitor the health conditions of individuals served and ensure quality services are provided. Administer medications and provide documentation as well as oversee and monitor staff members who administer medications. Provide prescribed medical treatment by physician’s orders and personal care services. Conduct assessments, implement health care services to be included in each Individual Service Plan (ISP), and respond to changes of condition. Provide training to staff members including universal precautions, infection control, and medication administration. Serve as liaison and advocate with other medical and professional service providers as well as work with external agencies to coordinate services and ensure consistent treatment of individuals receiving care. Provide training for individuals served with topics including self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation. Qualifications: Associate’s Degree in Nursing or related field, or state certificate One year of experience working with population served in a nursing capacity Current state LPN or LVN licensure for state of residence Current driver's license, car registration, and auto insurance Other certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training Ability to effectively communicate with staff, individuals served, and families Why Join Us? About the unit Our sub-acute floor is a 59 bed Medicare unit that has residents with multiple clinical issues including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac related issues needing monitoring. Nurses work closely with a variety of health care professionals including doctors, social work, dieticians, PT/OT, hospice, and spiritual support. Our sub-acute unit offers a well-rounded opportunity for learning, professional growth and personal satisfaction. Our Chronic Care floor is a 60 bed Medicare/Rehabilitation and long-term care unit and is a great place to learn basic med-surg, I.V. therapy & wound treatments nursing skills. Nurses on this unit along with host of healthcare professionals work to provide and implement the best treatment plan for residents. This fast paced and busy unit provide a great atmosphere for PT/OT residents to be motivated and reach their goals. Our Dementia Care floor is a 60 bed Medicare/Medicaid/Dementia unit. We provide care to residents with a variety of diagnoses. The dementia care is where we create a fullness of life through the joy of relationships, the art of caring and the spirit of living. This unit also works closely with a multidisciplinary team meets and discusses the resident’s plan of care. About the Facility Beaumont Commons – Farmington Hills Our facility has 3 floors and 9 unit (3 units per floor), we are able to provide care for long term care residents and post-acute rehab patients. The first floor has 59 beds with a focus on sub-acute rehab, the second floor has 59 beds which focuses on long-term care and sub-acute rehab and our third floor is a secured dementia unit. Nurse-patient ratios vary depending on the shift and the population you will be serving. We care for dementia residents, long term care and sub-acute rehab residents. With 179 beds, we accept but not limited to patient specialties, such as general rehab, neuro rehab, cardiac, respiratory and cancer. We also have various programs for residents to participate in multiple activities. Scope of work Licensed Practical Nurse (LPN) Under the direction of the RN/Mid-level Provider/Physician and in collaboration with other members of the health care team, contributes to the assessment, planning, implementation, and evaluation of patient care. Informs patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate, in coordination with the RN/Mid-level Provider/Physician. Documents care provided via computer/written patient record in a manner consistent with guidelines. Qualifications Required Graduate of an approved LPN program. Caseload is self-scheduled but communication of the clinician’s schedule to the nursing supervisor is essential. Communicates in case conferencing sessions. Effectively functioning as an involved team member to collaborate on best practices for the individual patient’s needs. Submits accurate, complete paperwork at the end of every week so that all medical records are intact and up to date. Uses the drop box if the office is already closed for the weekend. Notifies the supervising RN with requests for re-authorization of “more visits needed” prior to third party insurers authorized number of visits expiring. Suggests Community Services that may be available to the patient to assist in safe home care needs. Participates in the Performance Improvement Committee process as requested. Participates in staff meetings. Adjusts visits on the staff meeting day to accommodate this function. Participates in the growth of the office by being a willing preceptor for newly hired, same discipline employees. Maintains LPN licensure and actively seeks out educational experiences to enhance the practice of home care nursing for her/him and the benefit of the patients serviced. How you make a difference The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. Working under the supervision of RN(s), the LPN provides direction and guidance in patient education and activities that align with their education and demonstrated competencies. Key Responsibilities Under RN supervision, assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed. Implements treatment programs directed by Health Care Practitioners, administering medications, including IV therapy (if certified), and performing diagnostic tests according to established protocols and procedures. Collaborates in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions while responding to codes or health emergencies within standard guidelines. Assists in medical or minor surgical procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel. Attends mandatory staff meetings and training sessions, demonstrating a commitment to continuous professional development and adherence to policies and procedures in providing high-quality patient care. Additional Details We are looking for a full time Day LPN to join our clinical team at Macomb County Jail. This position works 7a-7:30p and EOW. Qualifications & Requirements Education Graduate from an accredited school of nursing. Experience Prefer a minimum of one (1) year clinic experience Licenses/Certifications Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
