Re-evaluate the patient plan of care at least every 90 to 180 days and when there is a change in the patient condition, after a hospital stay, and a time of discharge revising the Plan of Care as needed to achieve patient goals. Documents direct patient services, coordination and collaboration with physician and other disciplines or provides in the care of the patient, ensuring document is complete, and complies with acceptable home health standards and Agency policy. L. Documentation is timely and meets professional standards of practice, establishes compliance with state regulations and Medicaid conditions of participation. M. Reviews caseload and charts documentation to ensure compliance and assesses patient progress and effectiveness of care delivery in meeting patient goals. N. Hourly Wage: $21- $22.25 dependent upon date of certification Generous Shift Differentials 2nd shift: $1.50 3rd shift: $2.00 Weekends $2.00 We are looking for compassionate individuals to join our team as CNAs for the following shift needs: Full -Time 3pm -11pm 30 hours; e/o weekend $1000 retention bonus paid in two equal installments of $500 when hired for this shift Full-Time 7am - 3pm 30 hours; e/o weekend Part -Time 3pm -11pm 22.5 hours; e/o weekend required Part-Time 3pm -11pm 7.5 hours; e/o Sunday Part-Time 7am-3pm 22.5 hours; e/o weekend As a Certified Nursing Assistant , your main role will be to deliver care to our residents within a warm, comfortable, and home-like environment. Must be a CT State Certified CNA for consideration CNA Duties & Responsibilities: Assisting residents with activities of daily living, such as bathing, dressing, grooming, and toileting Documenting care provided and reporting any changes in resident health or behavior to appropriate staff. Engaging residents in meaningful activities and providing emotional support. Utilizing customer service skills to ensure that residents receive exceptional and meaningful care. Other duties as needed.
