As an integral member of the patient care team, the Registered Nurse supports the Chief Nursing Officer in leading the patient care team in providing for the safety, recovery and comfort of patients by implementing the nursing process (assessment, nursing diagnosis, developing the plan of care and the education plan; implementation of interventions, evaluation of interventions and revision of plan as needed); and provides education and advocacy. Practices in compliance with regulatory requirements including CMS Conditions of Participation, Title 22, CDPH regulations, DNV-GL/NIAHO standards, etc. The Registered Nurse supports department manager and director in quality improvement and growth of unit services; leadership in collaboration with co-workers and other departments; management of efficient services with meaningful and valuable outcomes. The In Home Services CNA is responsible for providing routine care and assisting participants or clients with activities of daily living, restorative and supportive care. Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties. Follows plan of care and assists participants with activities of daily living such as bathing, dressing, grooming, hair care, mouth care, eating, hand and toileting as assigned or needed. Documents care provided in accordance with established policies, guidelines and applicable regulations Carries out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews. This is a 12-hour shift position. The first 8 hours are compensated at the base rate, and the remaining 4 hours are paid at the applicable overtime rate. Responsibilities The Nursing Supervisor serves as the on-site administrative resource for the facility. The Nursing Supervisor collaborates, coordinates and communicates with administration, physicians and the entire management team to facilitate the delivery of quality patient care. The Nursing Supervisor serves as a role model by providing leadership and expertise and acts as a clinical resource for other members of the interdisciplinary team. The Nursing Supervisor responds to the needs and requests of others quickly and efficiently and assists others in completing tasks or duties for improved customer satisfaction.
