Utilization Management LVN At WelbeHealth, we are transforming the reality of senior care by providing an all-inclusive healthcare option to the most vulnerable senior population, while functioning as both a care provider and care plan to we serve. Our Health Plan Services team plays a critical role in our participants’ journeys, and our Utilization Management team ensures that we provide timely, quality, compliant, and cost-effective care to our participants. The Utilization Management LVN is accountable for the review and audit of authorization requests to ensure services meet standard medical guidelines. Essential Job Duties: Chart audits of items including but not limited to consult summaries, imaging results, and procedure summaries to determine if additional follow up services are requested Oversee departmental clinical inboxes, email inboxes, and fax queues to ensure appropriate handling of documents and authorization requests Identify, document, and correct inconsistencies and gaps in participants’ charts with authorizations in the UM system Review prior authorization requests for medical necessity and alignment with participants’ care plans, including routine office visits, procedures, and DME Job Requirements: Minimum of one (1) year of chart auditing or UM review experience Knowledgeable in areas of Medicare and Medicaid UM regulations Unencumbered LVN licensure Benefits of Working at WelbeHealth: Apply your clinical expertise in meaningful ways as we rapidly expand. A Certified Nursing Assistant (CNA) is responsible for providing routine nursing care in accordance with established policies and procedures and as may be directed by the Charge Nurse, RN Supervisor, Director of Nurses or Administrator, to assure that the highest degree of quality resident care can be maintained at all times. Certified Nursing Assistant (CNA) QUALIFICATIONS • Currently enrolled in a school for nursing assistants or have a license as a certified nursing assistant. • A minimum of 6 months experience as a certified nursing assistant or clinical experience while enrolled in a nursing assistant certification class. • Must possess a sincere desire to serve the ill, aged and infirm. • Ability to read, write and speak the English language. • Possess the ability to deal tactfully with personnel, residents, family members, visitors, and the general public. • Willingness to work harmoniously with other personnel. • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level they are currently functioning. • Must be in good general health and demonstrate emotional stability. • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility. Certified Nursing Assistant (CNA) GENERAL DUTIES AND RESPONSIBILITIES: GENERAL • Perform all duties as assigned and in accordance with facility’s established policies and procedures, nursing care procedures and safety rules and regulations. • Make resident rounds at the beginning of each shift and every two hours thereafter to administer quality nursing care. • Verify the identification of a resident before administering nursing care or serving food, to assure that appropriate care is being provided to the correct resident. Report missing ID bands to admission coordinator immediately. • Make beds, clean bedside and overbed tables. • Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Job Description The Emergency Department Registered Nurse is specially trained to provide prompt response and treatment for any patient presenting with any life threatening, emergent, or urgent medical need. Provides monitoring and therapeutic equipment necessary to provide specialized nursing care to acute or complex patients. Collaborates with interdisciplinary teams to develop and implement a plan of care for a specific group of assigned patients, ensuring the coordination of care between other disciplines, physicians, and support staff. Performs professional nursing duties that incorporate the psychosocial, psychomotor and age appropriate cognitive skills of the patient/family/significant other to assess, plan, intervene and advocate for the patient on an ongoing basis throughout the continuum of care. Will reevaluate patient response and outcomes, effectiveness of plan of care, and readiness for discharge and revise plan or make referrals as necessary. Acts as a role model and mentor to a variety of new graduate nurses, orientees and support staff. Responsibilities $25,000 Sign-on Bonus PROFESSIONAL CLINICAL PRACTICE: Provides patient care, reviews and administers patient medications, including intravenous titrations, invasive and arterial lines, cardiac monitoring, ventilator management, treatments and procedures in a safe, effective and age appropriate manner that meets scope of practice and standards of nursing practice. Recognizes change in patient's clinical conditions, taking appropriate action and documenting intervention. Appropriately responds to RRT/Code Blues/Code Whites as assigned. Effectively collaborates with the health care team to intervene, and advocate as necessary. Intervenes appropriately in instances of abuse, assault or neglect, or when adequate financial or social resources are not available. Makes appropriate referrals to resolve patient issues effectively. PLAN OF CARE: Develops long and short-term goals consistent with treatment plans, standards of practice, and the plan of care, incorporating patient preference and discussing plan of care with patient. Appropriately utilizes pain management resources and other interventions to provide pain relief. Provides ongoing assessments and documents the patient's physical and psychosocial condition, as well as response to care. Reviews patient test results, identifies critical results, and communicates in a timely manner consistent with the condition of the patient and correlates information into the plan of care. Consistently ensures nursing practice is evidence based, within scope of licensure and compliant with hospital wide standards of nursing care, including core measures. Ensures Infection Control procedures and HIPAA compliance. COLLABORATION: Collaborates with staff, physicians and members of the healthcare team, ensuring the coordination of care between other disciplines and support staff, focusing on efficient and effective emergency department throughput. Supervises, monitors and delegates patient care activities to appropriate members of the health care team, provides effective leadership and ensures that care is safe and of high quality. Acts as a source of information and articulately responds to questions, providing resources, guidance, and education about treatment plan, plan of care, and medications. Develops and implements accurate teaching and ongoing discharge plans. COMMUNICATION: Empathizes with patient and patient support systems, expresses compassion and understanding for the situation and demonstrates care which is appropriate for the age, and cultural, religious, gender identity, or ethnic background.
