Registered Nurse (RN) Nights & Weekends

Overview The Registered Nurse (RN)-Nights & Weekends is prepared to handle all on-going needs, in addition to urgent needs of patients and their families after hours in their homes or facility. The RN Nights & Weekends follows the established care plan of the patient while ensuring comfort care to the patient and support to family members. Location: Falls Church, Arlington, Alexandria Schedule: Every other weekend days , some nights and some holidays. COVID-19 vaccine required to be completed upon start. Responsibilities The RN Nights & Weekends visits patients with emergent, critical needs in a timely manner. The RN Nights & Weekends arranges for, teaches, and supervises nursing care of patients in home care and facility settings in collaboration with other members of the interdisciplinary team. The RN Nights & Weekends makes nursing assessments and uses critical thinking skills to provide care based on evidence-based practice, within scope of practice. May be asked to rotate to other regions/departments. Qualifications Experience Requirements: Must have one year of staff nurse experience in a comparable setting/unit. Hospice and/or palliative care experience is strongly preferred. Education Requirements: Must have a degree in Nursing from an accredited nursing program. Licensure and/or Certification Requirements: Requires licensure from the governing body of practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice; or licensure from compact state. Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia. A CHPN certification is a plus. Driver's License: Must have driver's license in good standing and access to a vehicle during working hours.

RN, Admissions - Telehealth

Overview The Admissions Registered Nurse - TeleHealth (RN) will provide virtual telehealth nursing assessments including the Initial Comprehensive Nursing Assessment as allowable under the Alternate Operations Process as well as clinical documentation review to determine eligibility, and hospice onboarding informational conversations and coordination support to the Referrals Center and Admissions Team as needed for the admissions coordination efforts for patients who are in pre-admit status via technology based platforms. Location: Falls Church, VA Hours: Varied hours and schedules available COVID-19 vaccine required to be completed upon start Responsibilities The Admissions RN - TeleHealth will compile, assess, synthesize, collaborate, document, and communicate information pertinent to the admission processes for Hospice patients including medical history, medication lists, advance directives, supply and DME needs, hospice eligibility, treatment plan of care and goals of care. The Admissions RN- TeleHealth will identify needs requiring on-site bedside assistance and will arrange such resources to meet the identified needs. Qualifications Experience Requirements Must have two years of public health/community health, med surg or related nursing experience. Experience in home care, hospice, and/or oncology nursing preferred. Education Requirements Must have a degree in Nursing from an accredited nursing program. A BSN degree is preferred. Required Certificates and/or Licenses Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice. Licensure from a compact state is also accepted. Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia. A CHPN certification is a plus. Registered Nurse (RN) Hospice Case Manager

Overview R egistered Nurse (RN) Hospice Case Manager works in collaboration with the Interdisciplinary Discipline Team (IDT) to foster a quality experience in accordance with the patient and family’s care plan and wishes. The RN Case Manager plans and manages the nursing component of the hospice caseload as well as the patient's illness and symptoms. He/She re-evaluates effectiveness of interventions and adjusts the care plan as needed. Location: Falls Church, VA Hours: 8:00am - 5:00pm - occasional holiday rotation COVID-19 vaccine required to be completed upon start Responsibilities The Registered Nurse (RN) Hospice Case Manager provides, coordinates, teaches and supervises the nursing care of hospice patients and families/caregivers. The RN Case Manager is responsible for supervising Licensed Practical Nurses, Certified Nursing Assistants, and Patient Care Volunteers. May be asked to rotate to other regions/departments. Certified Nursing Assistant- Hospice Inpatient Center

Overview Certified Nursing Assistant (CNA) - Hospice IPU provides a central role in the care of the patient and works to enhance quality of life, in addition to assisting with all activities of daily living. The IPU CNA works under the direction of the Registered Nurse (RN) to provide care and support to the patients and their families in the acute care setting. The CNA follows the care plan and communicates any needs or changes to the supervising RN. Location: The Adler Center - Aldie, VA Schedule: 3 (12) hour shifts - 7:00pm - 7:00am COVID-19 vaccine required to be completed upon start Responsibilities Under the direct supervision of a Registered Nurse, the IPU CNA provides a central role on the hospice and palliative care teams. Providing personal care for the patient and assisting with activities of daily living, the IPU CNA functions in a supportive role and functions as an extension of professional services while following written instructions under appropriate supervision, and specialized hospice training. May be asked to rotate to other regions/departments. Qualifications Experience Requirements Must have previous and recent experience as a Nursing Assistant, preferably in a hospice, assisted living or hospital setting. Personal care experience is desired. Education Requirements Must have a certificate of completion from an approved Nurse Aide training Program. In addition, registration is required by the appropriate Board of Nursing as a Certified Nursing Assistant.
