About the Role Delivers direct/indirect patient care to adult acute inpatient rehabilitation patient utilizing the nursing process through data collection and assessment; implementation and prioritization of patient’s problems and needs; and development/implementation/evaluation of a nursing plan of care. Focuses on the unique needs of the rehab patient while transitioning them from an acute/post-acute care setting to a community setting. POSITION’S ESSENTIAL RESPONSIBILITIES: POSITION REQUIREMENTS: Assesses/reassesses patient’s condition based upon observation of patient and interview of patient and/or family, physician’s diagnosis and data, diagnostic results, and relevant data from other healthcare providers.
