MDS Coordinator The MDS (Minimum Data Set) Coordinator/Nurse is an RN or LPN that conducts federally mandated assessments of the residents at a long-term care facility. MDS Coordinators are responsible for collecting integral data and compiling it into a thorough assessment to help determine the functional capacity with appropriate plan of care and to determine the reimbursement for all payer sources in relation to the RUG-IV 66 and RUG-IV 48 system established by the Centers of Medicare and Medicaid Services. East Moline Correctional Center is a minimum security adult male facility. The facility is located in northwest Illinois overlooking the Mississippi River. Included at the center are housing units, work camp, administration building, one segregation unit and a 12-bed health care unit. Assist in all aspects of a resident’s daily life, including activities of daily living, meal service, and social engagement. Appropriately communicate with branch clinical leadership when a resident’s needs change. Knowledge of how to approach and care for residents with dementia. Qualifications: An active CNA License in good standing. High School Diploma or GED. Willing to take additional training or education, which specific states may require. CPR and First Aid certifications are required. Ability to handle multiple priorities. Possess written and verbal skills for effective communication. Competent in organizational and time management skills.
