Position Summary: Licensed Practical Nurse (Memory Care)

As a Memory Care LPN, you play a vital role in delivering compassionate, person-centered care to residents living with Alzheimer’s or other forms of dementia.

You’ll provide clinical support, ensure safe medication management, and foster a calm, engaging environment where every resident feels valued and respected.

Key Responsibilities

  • Provide compassionate nursing care to residents within our Memory Care neighborhood

  • Administer medications and treatments in accordance with physician orders and state regulations

  • Conduct health assessments and contribute to individualized care plans

  • Monitor changes in residents’ conditions and communicate promptly with leadership and families

  • Support and mentor caregivers to ensure consistent, quality care delivery

  • Maintain accurate documentation and uphold safety and infection control standards

  • Promote a positive, team-oriented culture aligned with Charter Senior Living’s mission

Qualifications

License: Active LPN license in the State of Illinois (required)
Experience: Prior memory care or senior living nursing experience strongly preferred
Skills: Strong clinical, communication, and leadership skills

Schedule: Availability for 2nd and/or 3rd shifts (Part-Time)

Monitor the skin health of the resident; provide preventative skin care; administer wound treatments as ordered. Collect and submit specimens for laboratory analysis as ordered. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
