Under general supervision and direction of the Cardiac Surgeons and Heart Center Intensivists, and in accordance with the rules and regulations of Massachusetts governing Nurse Practitioner or Physician Assistant practice, the Advanced Practice Provider (APP) will be responsible for the care of patients on the Cardiac Surgical Service, primarily those patients that are admitted to the Cardiac Surgery/Heart Center Intensive Care Unit. • The APP will participate in daily rounds with nurses, other Advanced Practice staff, Intensivists, residents, and multidisciplinary caregivers; perform bedside diagnostic and therapeutic procedures, participate in the development and implementation of care plans, coordinate tests and consultations, implement treatment plans, communicate with referring physicians, perform patient education, and prepare patients for discharge, all while keeping Surgeons and Intensivists updated as required. • The APP maintains open lines of communication between Cardiac Surgeons, Residents, Fellows, PAs, Nursing Staff, Cardiologists, Heart Failure team, Intensivists, and patients/families. Promotes a collaborative relationship and effort between the medical staff and professional nurses toward continuity of patient care and efficient use of resources. Serves as a role model for professional APP practice. This NP specifically works as an NP in cardiac surgery, credentialed with Operative First Assist, and/or acts as a responding clinician for patients with ventricular assist devices. Weekend Baylor Registered Nurse (RN) FLSA Classification: Exempt Department: Clinical Reports To: Clinical Manager Direct Reports: N/A POSITION SUMMARY: The Weekend Baylor Registered Nurse (RN) is responsible for partnering with the Clinical Team to administer and coordinate skilled nursing care to patients requiring home care services. Services are provided in accordance with physician’s orders, under the direction and supervision of the Clinical Director, and in compliance with applicable laws, regulations, and policies of the organization . *The Weekend Baylor RN’s schedule is every weekend (Saturday & Sunday) from 7:00AM – 8:00PM, as well four (4) hours on Mondays to conduct follow-ups with providers and/or complete documentation. RESPONSIBILITIES: Clinical Care: Provides direct care and case management of assigned patients, including initial assessment and development of plan of care, as well as oversight and administration of prescribed medications and treatments. Time Type: Part time Department: PACE Primary Care - Nursing All Locations: 26 Sturgis Street - PACE Winthrop Position Summary: Job Description Summary A Registered Nurse holds ultimate responsibility for direct and indirect nursing care, including providing nursing care, health maintenance, teaching, counseling, planning and restoration for optimal functioning and comfort. Skills include demonstrating a comprehensive understanding of the department’s immediate needs and long term growth goals, showing initiative, using critical thinking to problem solve independently, exhibiting leadership qualities, and effectively engaging with the clinical team. Essential Duties and Responsibilities: Qualifications and Requirements: NeighborHealth opens our doors to people from all demographics and cultures. Pay range: $40/hr up to $60/hr based on experience Responsible for the general oversight, assessment, planning, and construction of the nursing care delivered to the participant. Assess a patient through obtaining history, performing appropriate tests, making or collecting observations from the participant and caregivers, and consulting with other members of the IDT. Teaches patients and families health care regimens. Explains and provides information regarding disease and wellness, tests, results, medications, diet, exercise, and other aspects of the plan of care to patients. Safely and effectively administers medications, tests and treatments as ordered. Manages the refills of all patient medications with appropriate authorization by the provider. Provide and coordinate health teaching required by individuals, families and groups to maintain the optimal possible level of health. Completes biannual and status change nursing assessments for an assigned panel of participants in accordance with Care Planning Policy and documents problems, goals & interventions in the participant’s care plan. Utilizes Assessment tools including but not limited to PACE Nurse Assessment and Medication Management to collect data. Baccalaureate degree in nursing preferred. Current Massachusetts license in nursing. Current BLS certification required. Minimum of 2 years experience in ambulatory, home health, hospital or sub-acute setting preferred, equivalent training or related experience considered. A minimum of one year experience working with a frail or elderly population. Prior experience in interdisciplinary team setting a plus.
