The Surgical Technologist III is involved in process improvement or quality initiatives within Perioperative Services. This position also assists the nursing and medical staff in providing care to patients undergoing surgical intervention and performing surgical scrub technologist duties; collaborates with nursing and other disciplines, when necessary, to revise the plan of care to meet the changing needs of the patient. Has constant contact with the Operating Room staff, physicians, clerical staff, anesthesia staff, and materials management. Essential Job Functions: Clinical Practice Independently performs surgical scrub role within the operative field and can scrub in open, laparoscopic, and robotic cases in all service lines, including trauma, thoracic, vascular, orthopedics, general, gynecology, urology, otolaryngology, plastics, and colorectal Applies knowledge of advanced surgical techniques, is highly collaborative with colleagues, and is considered a role model among peers Applies knowledge related to emergency and trauma situations and surgical procedures in the operating room Demonstrates infection control practices by utilizing proper aseptic technique while dealing with blood/body fluid Correctly uses all sterilizers and high-level disinfectors related to surgical procedures; documents appropriately and maintains competencies, as required In conjunction with Unit leadership, Charge Nurse, and Central Sterile Processing, demonstrates organizational skills in case preparation, case cart picking, and accurate room set-up for instrument and tray contents in scheduled cases. Sets up instruments, supplies, and equipment according to the surgeon’s preference cards, policies, procedures, and standards of care Acts as an exemplary mentor and teacher for students and staff orientees Operates and troubleshoots video systems as needed. Works in collaboration with Administrative Coordinators, Specialty Managers, Clinical Engineering, and In-house Vendors to maintain video systems Provides input in organizing and coordinating the effective use of personnel and materials Provides cost-effective care by the proper use of equipment and instruments. Demonstrates appropriate utilization of supplies based on assessed needs, standard of car,e and surgeon’s preference cards Demonstrates knowledge of immediate and long term outcomes of surgical activities, including, but not limited to: sponge, needl,e and instrument count, aseptic technique, proper handling of specimens, proper labeling and handling of medications on a sterile field, and care of electrosurgical active electrodes and implants Properly handles and cares for instrumentation In collaboration with Perioperative Educators, develops, organizes, and delivers continuing education topics and/or courses for operating room technologist colleagues and Central Sterile Processing staff Assists in performing care of the surgical patient, including: Helps the team to prepare room Helps open packs, supplies, and instrument sets Helps position patient Monitors sterile field Opens sterile supplies onto the sterile field before procedure Performs counts Handles specimen(s) Assists with emergencies Assures quality measures are met, such as the Surgical Care Improvement Project (SCIP) core measures Resource to Clinical Service In collaboration with Unit leadership and the Charge Nurse, determines individual patient/surgeon needs and adjusts instrumentation and supplies according to those needs Inventories and restocks supplies in individual OR suites and Specialty Service carts Responsible for reviewing and updating preference cards in collaboration with the specialty team Liaison to Central Sterile Processing (CSP) In collaboration with the Specialty Manager & Central Sterile Processing management team, acts as a liaison to CSP in the maintenance of specialty instrumentation necessary to enhance the delivery of quality patient care for assigned service(s) Perform unit-based in-services to peer groups on a bi-yearly basis for education and learning purposes Patient Flow Facilitation, Communication, & Control Desk Communicates with the oncoming shift any pertinent information regarding patients or unit activities Acts as a resource person to patients, visitors, physicians, & other staff Facilitates inter- and intradepartmental communication flow of the OR schedule Collaborates with the Scheduling Board Manager, Anesthesia Coordinator, Charge Nurse, and Perioperative management team in providing accurate and timely information regarding patient status and OR schedule Assists in efficient room turnover and case set-up Performs other duties assigned Leadership Consistently demonstrates leadership abilities in support of UCMC’s goals, including excellence in safety, quality, and the patient experience Performs the preceptor role in the training of students, orientees, and OR staff; Assists in the orientation of staff Serves as a mentor and role model Serves as a resource for float staff Actively participates in at least one department or medical center committee Contributes to creating a work climate that encourages positive staff morale, motivation, and commitment for all who participate in the care of patients Participates in a changing environment by being supportive and cooperative with changes at the unit level. Under the direction of an Attending Physician or a licensed independent practitioner, RN provides technically complex and high risk nursing care services to patients which may include one or more of the following categories: sedation, intravenous infusions, complex procedures, e.g., lumbar puncture, cardiac catherization; This may also include specialized monitoring during procedure and recovery phase; The RN may have Charge Nurse responsibility for his/her assigned unit. Essential Job Functions The RN is expert in assessing and treating and evaluating patient conditions specific to their work area, e.g., anxiety, pain, claustrophobia and underlying disease conditions. Due to the complex nature of pain, RN will evaluate and treat pain in a prompt, caring fashion and reassess after each intervention, charting per UCMC protocol. RN maintains hospital required nursing & ICU unit specific competencies needed to provide proficient nursing care; RN will provide continuity of care for infant, pediatric or adult patients based on area of specialty. This includes assisting in the care of critically ill patients during sedation/procedural encounters. Administering high-risk infusion and transfusion (i.e. chemotherapy medicines). Contributes to the Patient Services Sector/Outpatient Services Sector offering constructive suggestions for change and participating in the planning, implementation and evaluation of new and existing programs utilizing evidence based practice. RN uses nursing knowledge to anticipate and plan for patient care needs. The RN understands and applies safety principles and serves as a resource to other healthcare personnel on patient care. RN knows the potential risks and complications from specialty procedures and can assist in the initial treatment of adverse effects in their area of expertise. Under the Physician direction, the RN administers minimal / moderate sedation analgesia providing patients the ability to tolerate unpleasant procedures while maintaining adequate cardiorespiratory function, protective reflexes, and the ability to respond purposefully to verbal and /or tactile stimulation. As it is not always possible to gauge how a patient may respond to sedation, the registered nurse must possess clinical skills to rescue the patient whose level of consciousness progresses to deep sedation. Patient selection for sedation is based on established criteria developed through interdisciplinary collaboration by UCMC Critical Care Professionals; the RN will utilize a sedation assessment to determine a patient’s suitability for RN monitored minimal/moderate sedation by identifying potential risk factors for undesirable outcomes. The monitoring RN will conduct the assessment and include data from numerous sources, such as chart review, patient physical assessment and interview, and consultation with other health care providers as appropriate. The RN must have the ability to analyze patient data obtained during sedation to identify risks and/or complications throughout a procedure. Must possess excellent airway management skills to prevent adverse outcomes that could potentially occur during moderate sedation. RN identifies changes in patients’ condition and reports findings immediately to the physician. RN must comply with UCMC sedation monitoring documentation protocols throughout the patient encounter. The RN will understand the pharmacology of sedation medications and reversal agents and observes the patients for desired medication effects thereby avoiding medication reactions or complications; accurate documentation of the patients’ response is an integral component of the monitoring process. Patient education is an integral component for successful moderate sedation; The RN will provide pre-procedure preparation information and education, verbal and written post-procedure instruction. RN will assess for sufficient understanding of patient/families ability to process information. RN will work collaboratively with interpreter as needed and other multidisciplinary team members as needed. Computer proficiency: familiarity with UCMC Information Systems preferred Strong interpersonal skills; ability to handle a wide variety of public contacts, including conflict resolution and communicating both verbally and in writing with discretion and courtesy with patients, families and co-workers Requires the knowledge & ability to administer minimal and moderate sedation as prescribed and directed by the attending or licensed independent practitioner.
