Responsibilities The Nurse Practitioner delivers annual risk assessment in a residential setting or telehealth, within the scope of practice for a Nurse Practitioner, as delegated by the Collaborative Physician. Essential Duties and Responsibilities Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine all physical, mental, and social needs present at the time of the visit Takes history, examines, determines diagnoses. Provides written documentation of patient visit, per NCQA standards Takes patient vital signs, as necessary. Places case management referrals and communicates with PCP as necessary. Communicates with patients, caregivers, agency nurses, other providers and vendors as necessary to assure proper diagnosis. Registered Nurse (RN) - ICU/CCU Full Time Days An ICU/CCU nurse provides critical care to patients with severe conditions, monitoring their vital signs closely, administering medications, performing advanced life support interventions like CPR and defibrillation, and collaborating closely with physicians to develop and implement individualized treatment plans; essentially focusing on the most critical cardiac patients within an intensive care unit setting. Responsibilities: Continuously monitoring vital signs like heart rhythm (ECG), blood pressure, and oxygen saturation, identifying and responding quickly to changes in patient condition. Precisely administering complex cardiac medications, including emergency medications like antiarrhythmics and vasopressors, according to physician orders. Performing advanced cardiac life support procedures like CPR, defibrillation, and pacemaker insertion in emergency situations. Interpreting and managing invasive hemodynamic monitoring data like central venous pressure (CVP) and pulmonary artery pressure. Educating patients and their families about their heart condition, treatment plan, and self-care measures. Providing emotional support to patients and families facing critical cardiac situations. Qualifications: Active Registered Nurse (RN) license in Texas. BLS and ACLS certifications. Minimum 1 year of experience required. Ability to rapidly assess patient situations, interpret complex data, and make timely clinical decisions. Expertise in cardiac monitoring, advanced life support techniques, and medication administration. Effective communication with physicians, other healthcare team members, and patients/families.
