Position Overview: We are seeking a dedicated and detail-oriented Case Manager (CM)—a registered nurse committed to optimizing both patient outcomes and healthcare resources. In this role, you will identify patients who would benefit from case management services and coordinate care across the continuum by collaborating with patients, their support systems, and the multidisciplinary healthcare team. You’ll assess individual needs, develop and implement personalized care plans, monitor progress, and advocate for the services and support that enable patients to achieve the best possible outcomes—clinically, functionally, and financially.
