Key Responsibilities Develops a proactive plan of care to address identified issues to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness. Uses clinical tools and information/data review to conduct an evaluation of member's needs and benefits. Applies clinical judgment to incorporate strategies designed to reduce risk factors and barriers and address complex health and social indicators which impact care planning. Conducts assessments that consider information from various sources, such as claims, to address all conditions including co-morbid and multiple diagnoses that impact functionality. Remote Work Expectations This is a remote role with 25-50% travel required, candidates must have a dedicated workspace free of interruptions. This role provides comprehensive patient care from admission through discharge, with a focus on perioperative management in the Cardiac Surgical ICU. The APP will manage ventilators, sedation, hemodynamics, and diagnostics, and coordinate care with multidisciplinary teams. Clinical expertise in managing complex conditions such as shock, sepsis, respiratory failure, heart and renal failure, arrhythmias, and post-operative cardiac patients is essential. Responsibilities may include advanced procedures such as central and arterial line placement, pulmonary artery catheter placement, and point-of-care ultrasound. Additionally, the APP may sit on committees and participate in Quality and Process Improvement Initiatives. This is a 24/7 service requiring rotation through days, nights, weekends, and holidays, with regional cross-coverage at other hospital sites. ESSENTIAL DUTIES: Assesses the physical and psychosocial status of clients by means of interview, health history, physical examination, and diagnostic studies.
