Position Summary

  • Location: This role will be work at home with 25-50% travel within Southwest Michigan (Barry, Van Buren, Kalamazoo, Calhoun, Branch, St Joseph, Cass, and Berrien Counties).

Key Responsibilities

  • Develops a proactive plan of care to address identified issues to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness.
  • Uses clinical tools and information/data review to conduct an evaluation of member's needs and benefits.
  • Applies clinical judgment to incorporate strategies designed to reduce risk factors and barriers and address complex health and social indicators which impact care planning.
  • Conducts assessments that consider information from various sources, such as claims, to address all conditions including co-morbid and multiple diagnoses that impact functionality.
  • Uses a holistic approach to assess the need for a referral to clinical resources and other interdisciplinary team members.
  • Collaborates with supervisor and other key stakeholders in the member's healthcare in overcoming barriers in meeting goals and objectives, presents cases at interdisciplinary case conferences.
  • Utilizes case management processes in compliance with regulatory and company policies and procedures.
  • Utilizes motivational interviewing skills to ensure maximum member engagement and discern their health status and health needs based on key questions and conversation.

Remote Work Expectations

  • This is a remote role with 25-50% travel required, candidates must have a dedicated workspace free of interruptions.
  • Dependents must have separate care arrangements during work hours, as continuous care responsibilities during shift times are not permitted.

Required Qualifications

  • Minimum 3+ years clinical practical experience as an Registered Nurse (RN).
  • Must have active and unrestricted Michigan RN licensure.
  • Must be willing and able to travel up to 25%- 50% of the time in Barry, Van Buren, Kalamazoo, Calhoun, Branch, St Joseph, Cass, and Berrien Counties. Reliable transportation required. Mileage is reimbursed per our company expense reimbursement policy.
  • Demonstrates proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint 
  • Effective computer skills including navigating multiple systems and keyboarding 
  • Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually 
  • Excellent analytical and problem-solving skills 
  • Effective communications, organizational, and interpersonal skills
  • Ability to work independently 

Preferred Qualifications

  • 2+ years of experience in case management, discharge planning and/or home health care coordination experience.
  • Behavioral health experience.
  • Bilingual in Spanish or Arabic.
  • Certified Case Manager.
  • Additional national professional certification (CRC, CDMS, CRRN, COHN).

Educational

  • Associate's Degree required.

The typical pay range for this role is:

$46,597.00 - $101,228.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

Job Summary The Cardiac Intensive Care Unit (12 CICU) is a 16-bed adult cardiac medicine intensive care unit. The unit specializes in patients who have heart failure, acute MI, cardiomyopathy, pulmonary hypertension, congenital heart defects, bridge to transplant, as well as patients requiring interventions. Patients may require the following therapies: Swan-Ganz catheter with FICK calculations, intra-aortic balloon pumps, Impella devices, CRRT, or Therapeutic Hypothermia. 12 Medical Acute Care (12 MAC) is a 32-bed adult cardiac/telemetry/stepdown unit. 12MAC nurses provide specialty care for a variety of cardiology patients. Common diagnoses include heart failure, arrhythmias, acute coronary syndrome, and pulmonary hypertension. The patient population includes those with Implanted Ventricular Assist Devises (VADs) and those receiving prostacyclin infusions. Staff members collaborate with the inter-disciplinary team to provide the best care and work collaboratively as a team. This team consists of Clinical Nursing Director, CNS, Educator, Supervisors, Attending Physicians, Fellows, Residents, and Interns, Respiratory therapy, Physical Therapy, Social Work, and Care Management. The D. Dan and Betty Kahn Health Care Pavilion is a 12-story hospital that houses 264 private rooms capable of converting to intensive care; a state-of-the-art neurosciences center; and high-level, specialty care services for cardiovascular and thoracic patients, along with advanced imaging. Scope of work The hospital based Wound Clinician serves as a clinical resource and consultant for the acute care of in patients with disorders of the gastrointestinal, genitourinary, and integumentary systems. The care extends to those with acute and chronic wounds (for example, complex draining wounds, fistulae, pressure injury, vascular ulcers), ostomies, percutaneous tubes and continence disorders. Professional responsibilities are carried out through direct patient care, clinical consultation, education, research and administration. General duties/responsibilities · Utilizes the nursing process when directing or delivering care to patients with WOC needs including assessment, diagnosis, outcomes identification, planning and implementation (e.g., coordination of care delivery, health teaching and promotion, consultation), and evaluation. · Utilizes and maintains current physical, psychosocial, clinical assessment, and therapeutic communication skills to determine individual patient needs and expected outcomes. · Participates in committees or task forces related to wound, ostomy, and continence issues. Wound/skin care · Provides consultation for individuals with altered skin integrity (e.g., pressure injuries, draining wounds, fistulae) to develop individualized plans of care and attain expected outcomes. · Provides guidance to staff in implementing protocols to identify, control, or eliminate etiologic factors for skin breakdown, including selection of appropriate support surfaces. · Provides appropriate education to patients, caregivers, and staff regarding skin care, wound management, care of percutaneous tubes, and draining wound/fistulae management. · Provides consultation for negative pressure wound therapy, (including subsequent dressing changes) provides collaborative care which includes patient’s next care setting. · Validates pressure injury data collection for nursing quality indicators. WOC care by providing the following: · Formal or informal continuing education programs. · Orientation sessions. · Collaborative practice committees. · Nursing councils. · Attends continuing education programs related to WOC nursing to maintain current knowledge and skills.
