Description Job Description PeaceHealth is seeking a RN Navigator for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $48.52 – $72.78. Responsible for the coordination of patient cases for specific departments or service lines. In collaboration with the Medical Director(s), referring physician and interdisciplinary team(s), the RN Navigator coordinates disease-specific multidisciplinary conference; subsequent specialty consultations; and provides care as needed for patients and their families utilizing resources efficiently and effectively. Coordinates the development and performance of service line educational activities, program evaluation and outcomes research; acts as primary point of contact for service line patients. Th is a remote position for our new A-FIB fast track clinic – this position will need licensure in OR & WA – and will provide service system wide for this patient population Essential Functions Patient and Family Liaison - Act as a single point of contact for patients and families including telephone triage regarding service line symptom management. Assist patients and families in understanding their diagnosis, treatment options, and the resources available, by utilizing excellent interpersonal skills. Act as a liaison and primary point of contact between surgery team and surgical patients and families before, during and after surgical procedures to include scheduling of follow up appointment(s), giving discharging patients specialized instructions for surgical preparation, home care, lab requisitions and prescriptions. Perform post-surgery phone calls to evaluate care progress and answer any questions. Clinical Liaison - Collaborate with the service lines and notify surgeons and surgical team if there is a critical patient who requires stand-by preparation for surgery or specialized care. Collaborate with all disciplines (dietary, cardiac rehab, pharmacy, home health, etc.) to ensure that the patient is discharged in a timely manner; monitor inpatient length of stay (LOS) and collaborate with the various disciplines to expedite timely discharge. Reconcile all medication for the surgeons/PA’s and prepare all discharge instructions. Clinical review - Develop and maintains clinical guidelines, policies, protocols and documentation for practice in service line programs. Make rounds in service line surgical cases. Collaborate with staff nurses, review labs, write information for the surgeons and PA’s on the progress notes and report any abnormal findings to the surgical team that may need to be addressed. Perform chart review and collection of data required by the state and other entities regarding cardiac surgery outcomes. Educator - Develop patient and clinician education program, tools, and documentation. Reinforce patient education and direct patients and families to available resources and supportive services. Collaborate with nursing education regarding competencies. Facilitate a class for service line patients bimonthly. Provide preoperative teaching to inpatient “add-on” cases as needed. Ensure patients receive a treatment plan that is understandable and feasible and meets their specific needs, and coordinate timely scheduling of tests, pre-surgical procedures, specialty appointments and treatments, including initiating appropriate referrals (both internal and external) to community resources. Follow patients through the continuum of care, in collaboration with key stakeholders in the treatment plans.
