About the Role

As a Certified Nurse Practitioner, you will serve as a key clinical provider delivering comprehensive care in an outpatient primary care clinic while also supporting Skilled Nursing Facilities (SNFs) and Long-Term Care (LTC) settings. As the Everett clinic builds patient volume (anticipated opening March 2026), you will primarily assist with SNF and LTC coverage. This role offers variety, autonomy, and the opportunity to help build a new primary care practice from the ground up.


What You’ll Do

  • Manage a patient panel addressing acute and chronic primary care needs
  • Deliver thoughtful, high-quality care with a focus on prevention and long-term outcomes
  • Collaborate with SNFs, LTCs, and outpatient teams to ensure continuity of care
  • Educate patients and families on diagnoses, treatment plans, and follow-up care
  • Perform minor outpatient procedures per credentialing and privileging guidelines
  • Refer patients to specialists or additional services as appropriate
  • Pilot and streamline initiatives that improve care coordination across settings
  • Complete timely, accurate documentation
  • Partner with the practice manager to help launch and grow a new primary care clinic
  • Float between Everett and Bellingham clinic locations as needed
