Job Description: HSL provides enhanced housing with services to seniors living in its four affordable housing locations, and at partner housing sites, with the goal of improving quality of life and supporting independent living. At its core, enhanced housing with services is a proactive approach wherein resident services staff regularly and actively reach out to each individual resident to engage with them around their health and wellness, identify areas of need/risk and provide intensive, individualized case management and support as needed and desired by the resident. Vacation Cash-Out Option 3 Paid Personal Days 11 Paid Holidays Accrued Paid Sick Time Responsibilities Assess and coordinate healthcare services for program participants in collaboration with clinical and residential teams Communicate medical updates and follow-up needs to relevant staff, including the RN, Residential Manager, and Health Care Director Accompany and assist individuals with medical appointments as required by care plans and DDS standards Document all nursing interventions, health changes, and communications in TIER and other designated systems Review and support implementation of Individual Support Plans (ISPs), including self-medication, hygiene, and therapy routines Maintain and monitor medication and treatment orders; report side effects or concerns to appropriate parties Train staff on medication administration policies and ensure adherence to protocols Keep health records current and compliant with DDS regulations Share relevant health information with residential and day program staff to ensure continuity of care Participate in staff meetings, supervision, and mandatory in-service trainings Promote safety, dignity, and respect for all individuals supported Uphold confidentiality and professional standards in all interactions Collaborate effectively with the care team and contribute to a positive, team-based environment Perform additional duties or medical interventions as assigned by nursing leadership Qualifications Education & Experience: LPN or RN with current MA license. Associate's degree required 1 – 3 years of related experience Knowledge of medication administration procedures and familiarity with the Medication Administration Program (MAP) preferred. Skills and Knowledge: Valid US Driver’s License and clean driving record required.
