Responsibilities Are you ready to challenge the expected to deliver the extraordinary? Under the direct supervision of a collaborative physician, physician site leader, and practice manager, the Nurse Practitioner (NP) provides medical care to patients within a detailed scope of practice. The scope of practice identifies the range of clinical modalities and protocols agreed upon between the NP and collaborative physician. Within the ambulatory care setting, under the collaborative physician and scope of practice, the NP completes physicals; orders diagnostic and therapeutic modalities: e.g., laboratory, x-ray, EKG, etc.; performs therapeutic procedures such as injections, immunizations, suturing and wound care, and infection management; prescribes medications; instructs and educates patients and their families in proper healthcare, appropriate illness prevention, and home care; and documents in the patient chart progress notes, orders, etc. These aforementioned responsibilities are performed under the scope of practice to optimize clinical outcomes. The NP must be credentialed as a mid-level provider by same Hospital(s) in which the collaborative physician has admitting privileges. Exceptional communication and relationship-building skills are essential for establishing rapport with facility leadership and interdisciplinary team members. Additional responsibilities may include training new providers and assisting in the onboarding of new facility accounts prior to a permanent Trio provider starting. Assesses patients' health care needs, monitors patients' progress, and confers with physician practice group and health care team regarding variances from the anticipated plan of care. Provides basic assessment of patient's psychosocial, financial, cultural, and family situation. Coordinates social services for patients and families to address needs.
