Hospice Clinical Manager (RN)

JOB DESCRIPTION SUMMARY The Clinical Manager is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Manager is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel. This is for the Teal Team which is East King County but they will work at the Lynnwood office location daily. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES 1. Receives case referrals. Reviews available patient information related to case, including home visits, to determine hospice/home care needs. Assigns appropriate hospice personnel to case as needed. Conferences with MEDICAL DIRECTOR regarding any questions about an individual's eligibility for services. 2. Reviews and evaluates each case through a variety of means such as home visits, confer ences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate. 3. Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course. 4. Performs medical records management and workflow in EMR system 5. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care. 6. Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement. 7. Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director/Director of Patient Care Services and hospice personnel. 8. Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals. Assists in the orientation of new employees. 9. Assists Clinical Director/Director of Patient Care Services and other supervisory hospice personnel in the planning, implementation and evaluation of inservice and continuing education programs. Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range. 10. Complies with accepted professional standards and principles. 11. Assesses health needs of community and assists in the development of plan(s)/ program(s) to meet needs. Participates in public relation and community activities that promote the organization's role as an effective member of the hospice and health care delivery system. Promotes customer service orientation to all hospice personnel. 12. Participates actively in quality assessment performance improvement teams and activities. 13. Performs other duties and activities as delegated by the Clinical Director/Director of Patient Care Services. Registered nurse with current licensure to practice professional nursing in the state. 2. Graduate of NLN accredited school of nursing with a BSN degree is preferred or a minimum of one (1) year experience as a professional nurse within the last three (3) years. 3. Previous experience in hospice/home care setting and with two (2) years management or supervisory experience. Proven ability to work within an interdisciplinary setting. 4. Complies with accepted professional standards and practice. 5. HCHB and Pointcare experience preferred 6. Has excellent observation, good nursing judgment and communication skills. Understands hospice philosophy and issues of death/dying. 7. Understands principles of pain/symptom management. 8. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. 9. Complies and maintains current CPR certification. 10. Demonstrates excellent observation, verbal and written communication skills.

Job Title/Position : Hospice Weekend Visit Nurse (RN) Reports To : Clinical Director JOB DESCRIPTION SUMMARY : The Hospice Weekend Visit Nurse (RN) is responsible for filling in for RN Case Managers, On-Call RNs, and Admission RNs experiencing an overage of visits per day during the weekend. The Hospice Weekend Visit Nurse (RN) will provide clinical and psychosocial interventions for patients and their families, skilled pain and symptom management and collaboration with providers within and outside of the hospice team to maximize positive patient outcomes. The work of the Hospice Weekend Visit Nurse (RN) provides considerable consideration for independent assessment and judgement in crisis situations, utilizing and consulting with the Hospice Medical Director and / or Administrator On-Call as indicated. This will be two 12-hour shifts, Saturday and Sunday weekly. Please reach out with any questions. All shifts available at present and in Snohomish, King, and Pierce Counties. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES : Consistent availability by telephone and email during assigned shifts. Conducts urgent physical and psychosocial assessments of the patient and family during and after hours and determines appropriate interventions. Provides and maintains a safe environment for the patient. Provides professional nursing care by utilizing all elements of the nursing process. Provides skilled nursing care focusing on pain control and symptom management. Provides interim guidance to the patient and family regarding diagnosis, medications, treatments and progress. Documents patient care thoroughly and timely. Completes, maintains and submits accurate and relevant clinical documentation regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate. Communicates and collaborates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Works closely with members of the IDG to provide continuity of care and support for the patient and family. Collaborates with vendors to obtain DME, supplies and medications when needed after-hours. Collaborates with the hospital and other community residential staffing to adjust interventions and plans of care as needed. Conducts after hours hospice admissions as necessary. Participates in mandatory meetings as required. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-services. Fulfills the obligation of requested assignments. Performs other job-related duties as assigned. POSITION QUALIFICATIONS : Understands and is committed to the hospice philosophy of care. Functions effectively as a team member. Holds a comprehensive knowledge of medication, treatment and therapies for pain and symptom management. Visits patients wherever they call home. Consults with physicians, other team members and administrative staff as indicated. Relates to and cares for people from all walks of life in a calm, courteous and professional manner. Maintains client confidentiality in accordance with HIPAA laws and company policy. Graduate of an accredited school of nursing. Maintains current unencumbered license to practice in the commonwealth of Washington as a Registered Nurse. Preferably has two years of experience in hospice, homecare, hospital medical-surgical or oncology nursing. Understands and works within state and federal laws and regulations and nursing scope of practice. Understands documentation requirements and is able to complete documentation in a timely manner. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. Must maintain current driver’s license and automobile insurance, providing own transportation.
