Attend seminars or workshops to maintain up to date knowledge of activities services Attend administrative staff meetings. Wellness Coordinator | Assisted Living Administrator (RN or LPN) Position Summary The Assisted Living Administrator / Wellness Coordinator provides operational leadership and oversees resident wellness within an Assisted Living community that operates in close collaboration with an affiliated Skilled Nursing Facility. Key Responsibilities Administration & Operations Provide daily leadership and oversight of Assisted Living operations. Ensure compliance with all state and local assisted living regulations. Develop, implement, and maintain policies and procedures. Oversee census management, admissions, moveins, and discharges. Collaborate with leadership on staffing, scheduling, and operational planning. Ensure the community maintains safety, cleanliness, and regulatory readiness. Wellness & Care Coordination Oversee resident wellness programs and service delivery. Coordinate assessments, service plans, and ongoing evaluations. Collaborate with clinical teams, physicians, therapists, and external providers. Monitor changes in resident condition and coordinate timely interventions. Ensure accurate and timely wellness documentation. Staff Leadership & Performance Recruit, train, schedule, and supervise Assisted Living staff. Set clear expectations and provide coaching, feedback, and performance management. Foster a culture of accountability, teamwork, and resident centered care. Ensure staff competency, training compliance, and adherence to care standards. Resident & Family Experience Serve as a primary point of contact for residents and families. Address concerns promptly, professionally, and compassionately. Lead resident and family meetings as needed. Promote dignity, engagement, and quality of life for all residents. CrossCampus Collaboration (ALF–SNF) Maintain strong working relationships with Skilled Nursing leadership and clinical teams. Support smooth transitions between Assisted Living and Skilled Nursing levels of care. Align wellness practices, emergency protocols, and communication standards across the campus. Medicaid Coordination & Financial Eligibility Support residents and families through the Medicaid application and eligibility process. Complete, collect, and submit required Medicaid documentation in a timely manner. Partner with financial and administrative teams to ensure alignment on eligibility requirements and documentation status. Track application timelines and follow up on outstanding items to support continuity of care and appropriate placement. Qualifications Required Experience in assisted living, senior housing, or healthcare leadership. Knowledge of assisted living regulations and compliance standards. Demonstrated leadership and staff management experience. Strong communication, organization, and problem-solving skills. RN or LPN Preferred Experience working in a campus model with Assisted Living and Skilled Nursing. Familiarity with electronic health records and care documentation systems. Physical & Work Requirements Onsite presence required. Occasional evenings, weekends, and on call responsibilities. Ability to respond to emergencies and resident needs.
