Documents patient information in the medical record to include update vital signs, medications, test results, and patient concerns, etc. Maintains clinical and administrative knowledge base and designated skill set in order to meet established goals for daily call metrics, and defined individual/team goals. Understands insurance restrictions for durable medical equipment (DME) and supply ordering, lab work, referral needs, etc. Assists in the ordering or processing of DME and supplies and coordinates the scheduling of services (e.g. mobile phlebotomy) or referral needs on behalf of the clinical care team. Ensures care coordination and complete communication (closes the loop and follow up) between internal and external individuals, in an appropriate and timely way. Employee Health Accountabilities: Administers vaccines; reads PPD results, under the direction of licensed staff. Specimen management: obtain specimens for testing if applicable (including performing phlebotomy and anterior nares nasal swab collections), performs laboratory and screening tests, records results in EMR and reconciles with logs where appropriate, documents and reports findings to providers, performs all quality controls as per Point of Care Testing guidelines, prepares specimens for transport as necessary. Maintains work area, treatment area, and testing areas in a clean, safe manner; restocks with supplies and equipment as needed. Clerical Accountabilities: Manages office telephone systems, including answering calls and returning messages. Participates in responsive and proactive telephonic patient care, including care coordination with specialists, ancillary services, hospitals, labs, other home care agencies, and all other involved health services. POSITION SUMMARY: The primary purpose of your job is to provide a safe environment, give emotional and social support and attend to the resident's physical needs and comfort. Performs direct care to the residents as assigned, in accordance with the resident’s care plan, and as directed by your supervisor. Performs other duties as directed by representatives of the community. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the community's administration and Nursing Policies and Procedures. Education & Qualifications: Must be a graduate of an approved school for Nursing Assistants/Certified Nursing Assistants/State Tested Nursing Assistants and within the 120-day period allowing the graduate to work as a Nursing Assistant, under the laws of that state. Must take and pass the state licensing exam within the 120-day time period. Must be a high school graduate or equivalent. Must be at least 18 years old (if 16 years old, must currently be in high school.) Previous experience in long term care preferred but not required. Must be able to communicate in English and have the ability to follow verbal and written instructions. About the Role The Direct Care Worker (DCW) provides compassionate, hands-on support to clients in their homes. You will assist with daily living activities, promote comfort and safety, and support clients’ independence while following an individualized care plan.
