Ensuring services provided by other agencies are authorized by hospice or home health. Monitoring operational progress toward accomplishing operational and strategic goals. Ensuring appropriate data collection and regular, complete reports are received by the Governing Body. Ensuring adequate space, equipment and supplies are available. Ensuring actionable objectives are derived from evaluation of services and personnel. Ensuring that structure and systems promote interdisciplinary care Ensuring collaboration with agencies and vendors for effective management of services. POSITION DETAILS: Schedule: Days, 10-hour shifts in person (not remote) FTE: Part-Time, 40 hours per 2 week pay period, as of now JOB SUMMARY: The Care Coordinator, in collaboration with Gastroenterology specialty medical providers, performs case management and direct patient care to assigned patients. This includes point of care procedures, management of Gastroenterology and other medical devices, ongoing patient, family and staff education and coordination of care with outside community agencies. The coordinator also triages and manages patient requests and needs via telephone triage and patient messaging via EPIC. The coordinator also partners and supports continued growth of medical assistants to deliver well rounded care. The Gastroenterology division care for patients with general and complex diagnoses and symptoms. The Coordinator facilitates effective functionality of clinic operations, actively participates in Quality Management and acts as a provider extender within his/her scope of practice. The Care Coordinator performs additional team functions as required to maintain effective customer service. The coordinator role in Gastroenterology has a blend of hand on procedures, complex patient care coordination, education on disease management, triage, and in clinic support. We are looking for someone that is autonomous, collaborative, supportive of teams, educator, customer focused, and solution driven. The coordinator would be joining a large division with a collaborative partnership with Providers and ancillary support staff as well as an engaged and dynamic team.
