Provides oversight of all patient care services and personnel. Oversight includes making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring patient needs are continually assessed, and assuring the development, implementation, and updates to the individualized patient plan of care. Plans, directs, and participates in delivery of home health services to provide diagnosis-specific patient care, including coordination of services with providers, vendors, or facilities. Ensures the timely start of patient care within recommended regulatory guidelines. Processes and coordinates physician orders, staff assignments, and communications between direct care team members, the patient and their family, the following physician and primary care physician (as appropriate). Reviews and approves plan of care and evaluates proposed changes to the plan of care for clinical appropriateness. Integrates orders from all relevant physicians involved into the plan of care and ensures the orders are approved by the responsible physician. What is Required? Associates Degree in Nursing related field is required. 2+ years home care experience as a Registered Nurse in Home Health or Behavioral Health 1+ year supervisory experience Current state license as a Registered Nurse Proficiency with enterprise applications such as Workday and HomeCare HomeBase Proficiency with MS Office Suite (Word, Excel, Outlook) You will report to the Branch Director.
