Responsibilities Primary: Ability to fulfill all responsibilities of the Patient Care Specialist position as detailed in the specific Patient Care Specialist job description. Triage patients by checking vitals, performing blood draws and giving injections, etc. Observe patients, charting, reporting changes in patients’ conditions, such as adverse reactions to medication or treatment and taking any necessary action. Assist with lab testing and phlebotomy. Assist providers in preparing for minor procedures and physicals. Prepare and maintain supplies and equipment for treatment. Assist patients utilizing a Virtual Medicine Provider. Ensure accurate completion of patient charts in a timely matter and forwarding charts as appropriate on a daily basis. Arrange referrals to a specialist and obtaining pre-authorizations when directed by the Provider. The ability to maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in a positive work relationship with customers, co-workers and managers. Perform federal and non-federal DOT, BAT and Drug Screen exams. The ability to perform the physical, use of senses, cognitive, and environmental functions of the position, as specified on the physical demands. What you need to know about this position: Having a baby is one of life's most important experiences, and our Birthing Center provides a full range of delivery services to support needs before, during and after giving birth. Our Birthing Center team of experienced nurses and doctors welcome over 1,200 babies each year. A new Registered Nurse in this unit will have an hourly base rate starting at $30.70. An experienced Registered Nurse can expect an hourly base rate in the top 25% of nurses in the region. RN positions are eligible for applicable shift differentials and Sunday/holiday premiums. JOB RESPONSIBILLITIES: Restores and promotes patient’s health by completing the nursing process including but not limited to: a. Completes initial nursing assessment within 24 hours of admission or per Behavioral Health Unit/JMH (Johnson Memorial Health) policy; reassesses routinely throughout patient’s stay on the unit. b. Completes individualized treatment plan for each patient. c. Plans, implements, and coordinates nursing care activities for patients. d. Conducts therapies in accordance with patient’s diagnosis and needs. e. Performs treatment interventions following physician orders such as medication administration and medical treatment orders. f. Operates and participates in tele psych. i. Draws patient blood as ordered. Collaborates with physicians, multi-disciplinary team members, and outside health and social service agencies. Interprets the nursing and treatment needs of the patient for nonclinical staff assigned to the patient. Provides efficient discharge plans and obtains accurate histories at admission. Completes all required documentation relevant to patients in a timely, efficient manner. This includes but is not limited to: a. Admission paperwork b. Assessments (mental and physical) c. Progress notes d. Interventions e. Evaluations f. Discharge paperwork g. CQI+ data Protects patients and employees by adhering to safety standards, infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations. As needed, and using approved methods, assists in safely placing patients in seclusion or restraints and observes suicide/seclusion precautions including physical requirements for physical holds and safe transport of patients. Maintains continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs. Plans and implements nursing groups. Completes all required nursing staff competencies per program regulations in a timely manner. Orients or mentors new nursing staff as needed. Assures quality of care by adhering to therapeutic standards, measuring health outcomes against patient care goals and regulatory standards. Conducts patient chart audits. Establishes a compassionate environment by providing emotional and psychological support to patients and their families and friends. Participates in individual, group, and family psychotherapy and educational seminars as appropriate. Performs medication reconciliation. Ensures operations of equipment by completing preventive maintenance requirements, calling for repairs and evaluating new equipment and techniques. Provides feedback to supervisor on an ongoing basis regarding concerns, improvements, changes, etc. Maintains patient privacy and confidentiality and protects operations by keeping patient and program information confidential. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care, policy, procedures, standards of nursing practice, applicable contractual service agreements, state and federal regulations. · Performs all essential functions under the direction of the Supervisor and with guidance from the Educator, Preceptor or in collaboration with another Registered Nurse. · Performs ongoing, systematic collection and analysis of patient data pre – during – post treatment for assigned patients and documents in the patient medical record, makes adjustments and modifications to treatment plan as indicated and notifies Supervisor, Physician, patient’s primary nurse and others as may be indicated. · Assesses, collaborates and documents patient/family’s basic learning needs to provide initial and ongoing education to patients and family. · Recognizes aspects and implications of patient status that vary from normal and reports to or collaborates with appropriate health team members for input including but not limited to Supervisor, appropriate physician, and the contracted facility primary Nurse. · Directs and provides safe effective patient care for assigned patients as prescribed for all modality specific treatment procedures. · Assesses daily assigned patient care needs and collaborates with direct and ancillary patient care staff as needed. · Initiates and coordinates communication with FKC and Non-FKC dialysis providers and appropriate contracted facility personnel as needed to provide continuity of patient care. · Performs the implementation, administration, monitoring, and documentation of patient's response to prescribed intradialytic transfusions, including appropriate notification of adverse reactions to physician and appropriate blood supplier. · Administers medications as prescribed and documents appropriate medical justification and effectiveness. · Initiates and assists with emergency response measures. · Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned. · May be asked to provide specialized nursing care instruction to hospital/facility staff as stipulated contractually. · May be assigned to assist in an Outpatient facility on an as needed basis. · Required to complete CAP requirements to advance. · Performs all other duties as assigned by Supervisor. What you need to know about this position: Our Medical Surgical floor is an 18-bed unit with a nurse-to-patient ratio of 1:5-6. Patients on this unit could include those recovering from surgeries, bariatric procedures, and strokes or being treated for other medical conditions or receiving renal dialysis. A new Registered Nurse in this unit will have an hourly base rate starting at $30.70. An experienced Registered Nurse can expect an hourly base rate in the top 25% of nurses in the region.
