Penn Medicine Hospital at Home (H@H) is a virtual acute care program that utilizes state of the art technology to provide innovative clinical care to acute care patients in their home. The H@H program allows patients to receive hospital-level care in the comfort of their own homes. This model is designed for patients who are clinically stable enough to be treated outside of a traditional hospital setting. This care model includes monitoring, overseeing treatments, and coordinating with the interprofessional teams to ensure safe and effective patient recovery outside the traditional hospital setting. The team of H@H clinicians are onsite at Penn Rittenhouse campus, reporting to the Penn Center for Connected Care, and working collaborative with an interprofessional team of clinicians. The H@H clinicians are also onsite at Penn Medicine hospitals, identifying and enrolling patients into the H@H program. Regardless the physical site each day, the H@H team work closely to ensure effective communication, coordinated care, and superior patient outcomes. Clinicians will deploy software technology, hardware technology, and trended data analysis to care for the H@H patients, in collaboration with the Penn Medicine at Home team, who are in the home. The H@H clinicians are experienced in hospital acute care with a virtual care subspecialty skill set. In addition to the knowledge, skills, and abilities required to function as a bedside clinician, the H@H clinician must possess high-level skills in communication, collaboration, decision-making, systems-thinking, and computer literacy, necessitated by the remote nature of the practice environment. The H@H clinician requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. The H@H clinician may require long periods of continued sitting or standing at a desk in an office setting. The H@H clinician requires interprofessional communication via two-way camera technology. The H@H clinician must be confident, composed, responsive, and approachable when diligently interacting with patients, families, and interprofessional team via two-way audio-video technology. Summary: The Clinical Nurse 2 Colleague is an RN that practices as a full partner on the health care team and consistently demonstrates safe practice within the organizational professional practice model. The Clinical Nurse 2 Colleague possesses the knowledge, skills and the attitude to effectively meet standards for competent professional nursing practice as specified in the Clinical Nurse 2 position description addendum. The Clinical Nurse 2 Colleague exhibits UPHS organizational behavioral competencies which reflect its mission and are embedded in the professional practice competency domains. The Clinical Nurse 2 Colleague independently provides patient care through the application of the nursing process and accepts accountability for the nursing care of assigned patients. Assigned patients may range in age from infancy to the elderly. The Clinical Nurse 2 Colleague demonstrates within the competency domains of Continuous Quality Improvement, Evidence Based Practice and Research, Leadership, Person and Family Centered Care, Professionalism, Safety, Technology/Informatics, and Teamwork (Definitions listed below). The Clinical Nurse 2 Colleague works closely with the interprofessional health care team to facilitate the coordination of care across the continuum. The nurse focuses on expanding the knowledge and skills necessary to provide individualized care based on physical, psychosocial, cultural, educational, safety, and age appropriate considerations of assigned patients. The Clinical Nurse 2 Colleague seeks feedback in assuming leadership roles and provides feedback for improved clinical practice. The nurse demonstrates how nursing practice impacts the organizational vision, mission, and goals and the care delivery system. The nurse complies with all regulations and standards of regulatory and accrediting bodies. Evidence Based Practice and Research: The nurse evaluates and integrates best current evidence with clinical expertise and patient and family preferences and values for the delivery of optimal health care and system effectiveness. Leadership: The nurse effectively collaborates and applies innovative, systems thinking to engage in systematic, evidence-based problem solving and decision making to promote effective changes within a complex care delivery system supporting the vision of Penn Medicine. Person and Family Centered Care: The nurse recognizes the patient (or the patient’s designee) as the source of control and a full partner in providing compassionate and coordinated care based on respect for the patient’s preferences, values, and needs. Professionalism: The nurse demonstrates a commitment to the nursing profession through lifelong learning, adherence to ANA’s Code of Ethics for Nurses, participation in a professional organization and advancing community outreach. Safety: The nurse minimizes risk of harm to patients, families, providers and self through system effectiveness and individual performance. Technology/Informatics: The nurse utilizes appropriate information and technology to communicate, manage knowledge, mitigate error, and support decision making across the continuum. Teamwork: The nurse effectively engages in the process of cooperation, coordination, and collaboration in an effort to provide for safe, quality outcomes for patients within inter and intra-professional teams, including virtual teams. Credentials: Must have Bachelor's Degree in Nursing (BSN) from an accredited school of nursing, at time of hire. Requires 18 months of hospital acute care experience; prefer 3 years of hospital acute care experience. Must possess active Pennsylvania RN License, at time of hire. Must possess active BLS as a healthcare provider as per the American Heart Association at time of hire. Onsite Penn Medicine Rittenhouse campus.
