Maintains quality care that is consistent with company and regulatory standards. May assume responsibilities of daily operations of the facility in the Personal Care Administrator’s absence. Essential Duties and Responsibilities: Supervises the assigned Assisted Living /Personal Care Unit in order to provide quality nursing care to residents, and coordinates total nursing care for residents as illustrated by the following: 1. Determines work procedures, prepares work assignments, and expedites workflow. 2. Assures compliance with established standards, practices, and governmental regulations. 3. Establishes and adjusts work assignments as required to insure adequate coverage. 4. Participates in regular staff meetings for communication, reviews and/or educates on policies, grievances and support planning. 5. Assists in rehabilitation, occupational, and physical therapy programs. 6. Conducts rounds according to established schedule; documents resident progress, changes in condition, etc. 7. Assesses residents and analyzes resident information to develop resident care plan. 8. Provides routine, basic health care, and assistance with activities of daily living for all residents. 9. Performs resident treatments designated to be done by a licensed nurse. 10. Administers and records medications, records treatments, and reports any untoward reactions/side effects; monitors resident self-administration of medications. 11. Assists physicians on visits, examinations, and procedures; carries out doctors’ orders promptly and accurately. 12. Requests physician services, including timely orders, progress notes, certifications, and physical examinations. 13. Orders medications from pharmacy. 14. Schedules appointments for consultations, physical examinations, x-rays, laboratory test, and other ordered services; maintains appropriate records of services provided by consultants. 15. Records and reports changes in residents’ conditions. 16. Responds to residents, families, staff, visitors, and volunteers, and refers their concerns to appropriate staff. 17. Administers resident and staff intra-muscular medication, as may be required by EverTrue policy. 18. Responds to emergencies; assesses the condition of the resident; decides on type of medical attention; calls appropriate ambulance service if needed; notifies family and/or designated party when resident is transferred or admitted to hospital; notifies physician and family when death occurs; RN may pronounce as ordered. 19. Communicates status of residents to incoming shift personnel; receives communication from off-going shift personnel; prepares 24-hour nursing report; communicates with physicians regarding changes in resident conditions; transcribes, clarifies and implements physician’s orders. 20. Counsels residents and families; gives advice when approached; makes referrals to other departments. 21. Maintains residents’ medical files; records and documents assessments as necessary per residents’ health status and as required. 22. Completes employee and resident Incident Report form, as applicable. 23. Ensures that residents are aware of the services that are available; works with home health/hospice care services that residents may be receiving. 24. Responds to in-house and incoming phone calls and alarms. 25. Encourages residents’ participation in planned activities; assists as necessary. 26. Interviews and hires LPNs, Personal Care, and Medication Technician applicants; collaborates with Personal Care Administrator/Assisted Living Administrator on selected candidates for hire. 27. Directs, organizes and supervises the nursing staff on the unit. Disciplines staff when appropriate. 28. Develops and presents employee performance evaluations in collaboration with Personal Care Administrator/Assisted Living Administrator. 29. Guides, educates and mentors members of the unit’s team to develop skill sets to provide quality patient care. 30. Builds and maintains good staff rapport and morale. 31. Assists to analyze new competencies needed to impact acuity of current patients, ability to care for patients and admission of patients. 32. Provides ongoing staff education and competencies to professional and non-professional staff. 33. Initiates and promotes facility’s QAPI efforts. 34. Maintains open and honest communication with facility’s leadership and employees. 35. Ensures all associates in the Healthcare Center understand, embrace, and execute expectation of high level of customer satisfaction from internal and external customers. 36. Facilitates teamwork at the local level with facility and corporate support teams. 37. Positively influences the behavior of all team members. 38. Demonstrates the support of the community’s and organization’s strategic direction and positively communicates that direction to the nursing staff. 39. Attendance at all meetings assigned. Effectively communicates, implements, and ensures execution of any and all expectations and/or changes reviewed. Qualifications, Knowledge, Skills & Abilities: Graduate of an accredited school of nursing; current State license to practice as a LPN; maintenance of license by performing necessary continuing education requirements. Certified as a medication administration trainer or trainer certification obtained within 120 days of assumed role. A minimum of three (3) years' experience in related nursing positions preferred. Proficient reading, writing, grammar, and mathematics skills; proficient interpersonal relations and communicative skills; knowledge of drug interactions, side effects, and incompatibilities; decision-making skill; proficient assessment skills; knowledge of hazardous materials, bloodborne pathogens and infection control procedures; current CPR Certification ( or eligibility and pursuit of certification); ability to sit, stand, stoop, lift, bend, reach, push, and pull items weighing up to 50 lbs.; visual and auditory skills.
