Nurse Navigator-General Surgery (Harrisburg)

UPMC is hiring a Nurse Navigator for our General Surgery department in Harrisburg! This is a full time, day shift position with NO weekends and NO holidays required! This position serves as the nurse navigator for multiple specialties including otolaryngology, plastic surgery, with a primary focus of colon and rectal surgery. This position will be responsible for the oversight of NAPRC accreditation for the colon and rectal group, as well as nurse navigation for inpatient and outpatient care. The nurse navigator will assist in coordinating and presenting for multiple cancer conferences. Purpose: To function as a member of an interdisciplinary team, providing expert care related to wellness, prevention, illness and rehabilitation. Functions as an advocate for the patient and their family and acts as a liaison for resources and services. Serves as a clinical resource and consultant to members of the healthcare team and provides staff development. Participates in the evaluation of patient care and specialty services and programs through quality improvement activities. Responsibilities: Collaborates with the interdisciplinary team in guiding the patient and family in regard to diagnostic and treatment options. Provides emotional support specialized patient care, support and educational needs. Assumes the role of care coordinator and patient advocate throughout the process.
