Department Overview Acute Care (5A) and Family Medicine (5C) is a conceptually contiguous Family Medicine and General Medicine Acute Care unit with 14 beds on the Family Medicine unit and 23 beds on the Acute Care unit. The patient population is adults who have a variety of medical diagnoses such as Diabetes, End Stage Renal Disease, End Stage Liver Disease, Gastro-Intestinal Disorders, Adult Cystic Fibrosis, chronic and acute pain, infection, diagnostic testing, and psychiatric patients with a medical overlay. Department Overview The Nurse Manager of the Medical ICU (MICU) and Critical Care Float Pool (CCFP) is responsible for the operational oversight of the 16-bed MICU as well as the registered nurses who are members of the Adult CCFP. The nurses of the 7A Medical Intensive Care Unit (MICU) provide patient and family centered care to the patient populations that include: complicated BMT patients, end stage liver disease, end stage renal disease, respiratory failure, sepsis, GI bleeding, palliative care, drug overdose, post Code Blue resuscitation, and complicated diabetes management. Department Overview The OHSU South PACU provides 24/7 perianesthesia care to adult patients as well as some pediatric patients. The department cares for Ambulatory, Acute Care, and Critical Care patients arriving from home, the emergency department, or from the In-Patient setting and prepares them for surgery or other procedures. We also recover patients from anesthesia after their surgeries and transition them to the inpatient setting or discharge them home. OHSU is a Magnet designated hospital, a Level 1 Trauma Center, and the only Academic Medical Center in the state of Oregon. Function/Duties of Position The PeriAnesthesia nurses are responsible for assessing patient and family needs, planning and coordinating care, and for consultation of interdisciplinary services. Procedures and treatments provided in the PeriAnesthesia Department include but are not limited to: placing venous access, drawing bloodwork, medication administration, fluid and blood administration, radiography, ultrasonography, intubation, and cardiopulmonary resuscitation. Patient population and diagnoses vary from minor to critical to include both elective and emergent surgeries. The PeriAnesthesia Team works to determine level of care based on patient stability, discharge appropriateness, and resource and referral allocation. Department Overview The unit's dual mission is primarily focused towards providing robust nursing services for inpatient acute care epilepsy diagnostic monitoring (EMU) and non-acute care inpatient/outpatient clinical translational research (CTRC) services. In addition 10D also serves as an overflow unit for various hospital acute care patients. We take pride in providing outstanding participant & patient/family centered care in a collegial dual mission ambulatory and acute care setting where attention to detail, quality provision of care, and patient experience is our expertise and focus Patient Population Will Include But Is Not Limited To Epilepsy patients: Nursing care for patients for patients with seizure disorders admitted for epilepsy monitoring and surgical diagnostic intervention. JOB DESCRIPTION Job Summary Provides screening, preventive primary care and medical care services to members - primarily in non-clinical settings where members feel most comfortable, including in-home, community and nursing facilities and “pop up” clinics. Strives to ensure member progress toward desired outcomes and contributes to overarching strategy to provide quality and cost-effective member care. Essential Job Duties • Provides general medical care and care coordination to various and/or specific patient member populations – adult, women’s health, pediatric, and geriatric. • Performs comprehensive evaluations including history and physical exams for gaps in care and preventive assessments. • Addresses both chronic and acute primary care complaints, and demonstrates ability to ascertain medical urgency. • Establishes and documents reasonable medical diagnoses. • Seeks specialty consultation as appropriate. • Orders/performs pertinent diagnostic laboratory and radiology testing for the medical diagnosis or presenting symptoms; works within an environment of limited resources and therefore uses diagnostic tests judiciously and appropriately. • Understands when a member's needs are beyond their scope of knowledge and when physician oversight is needed. • Creates and implements a medical plan of care. • Schedules appointments for visits when appropriate. • Provides post-discharge coordination to reduce hospital readmission rates and emergency room utilization. • Performs face-to-face in-person visits in a variety of settings including in-home, skilled nursing facilities, and public locations. • Performs face-to-face visits via alternative modalities based on business need, leadership direction and state regulations. • Orders bulk laboratory orders to target specific member populations. • Performs alternating on-call coverage to triage any urgent lab results and pharmacy inquiries and develops appropriate plans of care. • Participates in community-based “pop up clinics” to build relationships with communities, and address gaps in health care. • Drives up to 120 miles a day on a regular basis to a variety of locations within the assigned region.
