Case management involves creating a Plan of Care for patients and following physician orders that lead to outcomes of increased patient independence and satisfaction. The RN for this position enjoys working with a team of supportive clinicians as well as being disciplined to ensure the highest level of care. Responsibilities: Assumes responsibility for assigned patient’s care from admission through discharge by effectively executing patient’s Plan of Care. Collaborates with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources. Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes. Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals. Participate in on-call rotation as reasonably assigned. Overview POSITION SUMMARY: The Nurse Residency Site Coordinator plays a crucial role in the transition to practice programs at their facility or service area. The Site Coordinator ensures the transition to practice program is consistently operationalized at the facility site. The Site Coordinator provides consultation to the education departments and helps develop and monitor education plans for residents participating in the program. The Site Coordinator also functions as a facilitator in the ministry-wide transition to practice and professional development programs. Qualifications REQUIRED QUALIFICATIONS: Education: Bachelor’s degree in nursing Experience: 3 years of clinical experience Licensure/ Certification: Basic Life Support Certification (AHA); Nursing Professional Development - Board Certified Certification (NPD-BC) within 1 year of hire Other Skills/ Knowledge: Excellent interpersonal and communication skills.
