When plan is in place, notify provider establish and determine anticipated readiness for discharge, keeping patient/family/so informed and documenting such in the EMR. The Care Coordinator collaborates with the team to assist the Multidisciplinary Team in providing discharge planning activities to assist in expediting a patient’s discharge as part of the care transitions process. Participates in the yearly written evaluations of nursing assistants and documents accurately their strengths, weaknesses and the steps that need to be taken to improve their performance. 14. Assists Staff Development with orientation and on-the-job training for new nursing personnel as needed. 15. Collaborates with nursing supervisor in documenting disciplinary concerns for nursing assistants on designated shifts, including reprimands, oral and written warnings, and corrective action recommendations. 16. Notifies nursing supervisor, resident’s physician, and family of a change in the resident’s condition or an accident/incident involving the resident. 17. Assure timely completion of all aspects of resident assessment process and timely transmission for federal and state agencies. 14.
