Day Center CNA

The Day Center CNA ( Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care. Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting. Document care provided in accordance with established policies, guidelines, and applicable regulations. Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews. Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed. Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner. Collaborate with multidisciplinary teams in care planning for participants. Monitor appetite and look for changes in functional ability and skin condition. Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary. Report any variance, accident, or unusual occurrence to the appropriate personnel. Assist with loading and unloading of participants from vehicles. Assist the transportation department with participants’ take-home supplies. Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities. May rotate into In-Home Services as a Homecare CNA. as deemed necessary by the Center Manager. Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed. Attend in-service and continuing education programs as scheduled and necessary. Responsibilities Our Medical Assistant greet our patients, obtain their medical histories and verify patient information Prepares patients for physical examinations, including taking vital signs and performing all required testing Provide assistance during examinations and treatment Perform ancillary testing and tasks as directed by the Center Medical Director (including but not limited to PFT, EKG, injections, audiograms, blood draws, etc.) and as certified Performs DOT and Non-DOT drug and alcohol testing In addition to Medical Assistant duties, you will be cross trained to perform Front Office duties including greeting patients, obtaining authorization, check in/out, communication of wait times, etc. Qualifications High school graduate or equivalent Graduate of an accredited Medical Assistant program, training as a medical assistant, or military medical specialist with current credentials in the state of employment Ability to work effectively in a dynamic, clinical office environment Certified Medical Assistant (CMA, RMA) preferred but not required Prefer applicants who are conversationally fluent, can read and write in both English and Spanish This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management. This position is eligible to earn a base compensation rate in the state range of $21.00 to $25.47 hourly depending on job-related factors as permitted by applicable law, such as level of experience, geographic location where the work is performed, and/or seniority.
