Overview Direct Support Professional III - LPN Seven Hills Neurocare, an Affiliate of Seven Hills Foundation The Direct Support Professional III- LPN is a Licensed practical nurse who is a direct service position providing individual medical support and education to individuals in assigned programs advocating for all medical needs of assigned program participants focusing on optimal health for all individuals. Hours assigned by supervisor; on call responsibilities. With advanced qualifications or significant experience, this role will provide specialized or high-level support, while maintaining quality, service excellence and ethical standards. Pay: $40.00/HR Direct Support Professionals are provided a paid New Employee Orientation and receive training on CPR/First Aid, Safety Care, Defensive Driving, and Medication Administration Program (MAP). Personal PTO - (Minimum 20-hour regular part-time schedule - does not apply to per diem) Responsibilities Assess and coordinate healthcare services for individuals, ensuring timely follow-up and communication with relevant clinical and residential staff Accompany and assist individuals with medical appointments in accordance with DDS guidelines Document all nursing interventions, health updates, and medication changes in Electronic Medical Records (EMR) and communication logs Monitor and maintain current medication and treatment orders, report side effects or adverse reactions as appropriate Review and support implementation of Individual Service Plans (ISPs), including self-medication, behavior plans, hygiene, and therapy programs Train residential staff on medication administration and health protocols Ensure up-to-date and compliant health records in line with DDS standards Communicate medical recommendations clearly to all program staff involved in the individual's care Maintain safety and monitor for health-related risks within the program Participate in staff meetings, mandatory trainings, and clinical supervision with the Health Care Director or program RN Perform medical interventions and additional responsibilities as assigned Uphold confidentiality, dignity, and respect for all individuals receiving services Foster a collaborative, supportive, and professional environment in alignment with Seven Hills Foundation values Qualifications Education & Experience: High school diploma, GED, or HiSET required. Licensed practical nurse required (LPN) 1-3 years of related experience preferred. CPR/First Aid and MAP certification required (paid training provided if needed). Skills and Knowledge: Strong communication and organizational abilities. Compassionate and patient approach to care. Commitment to fostering inclusion and independence. A Licensed Practical Nurse bears full responsibility for the quality of health care they provide to the residents at Seven Hills Pediatric Center at Hopedale. A Licensed Practical Nurse may delegate nursing activities to assigned health care personnel. The delegating Licensed Practical Nurse shall bearfull responsibility for making an appropriate assignment, adequately teaching, directing and supervising the delegate(s), as well as the outcome of that delegation; all within the parameters of their generic and continuing education and experience. Provides quality care to children and young adults in accordance of the Nurse Practice Act of Massachusetts. Responsibilities •Assess and coordinate healthcare services for Program Manager. Institute appropriate follow up and communicate all medical issues to the program Residential Registered Nurse and any other appropriate Seven Hills Pediatric Center at Hopedale staff members.•Assist individuals in designated programs to their medical appointments and with transportation as recommended by the Program Manager, Registered Nurse, or the Nurse Practitioner.•Document all nursing interventions and changes affecting the children in appropriate areas.•Review the plans of care for current knowledge of programming as required by the Department of Children and Families and/or other agencies.•Maintain current medication and treatment orders noting any side effects and/or adverse effects and report to appropriate individuals: MD, NP, Program Manager or program Residential RN, and other designated Seven Hills Pediatric Center at Hopedale staff. Assist in training staff in medication policies and procedures.•Maintain health records according to various government agencies as required. Ensure that information is communicated to residential staff and school program regarding recommendations and treatment.•Consult with residential Registered Nurse for supervision and direction as needed to ensure appropriate medical health care is provided for participants.•Attend staff meetings to present all current medical/health concerns and any upcoming medical appointments to team.•Complete all mandatory in-service education in a timely manner and attend any educational programs at the recommendation of the Program Manager.•Implement procedures and services including medical interventions, or additional duties as assigned by the Program Manager or residential Registered Nurse.•Ensure that residents are sufficiently observed to maintain their safety and optimal health. Qualifications •LPN Nurse with current Massachusetts license. •One year of experience working with medically fragile children. •Valid driver’s license required.
