Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care. Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.). Provides appropriate support at time of death and perform bereavement assessment. What is Required? Communicates frequently with the Branch Director or Administrator on key performance metrics. Remains available for field work, as needed. What is required? Associate degree in a Nursing related field, Bachelor preferred 2 years of home care experience in Home Health or Hospice environment 1 year of supervisory experience required Current, unrestricted RN license valid for the state of work Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office Able and willing to travel up to 50% as needed to support business and patient needs. Develops and clinically manages the Home Health Aide’s care plan according to the patient’s evolving personal are needs. Coordinates care, collaborates and communicates with other members of the interdisciplinary team to assure appropriate and smooth flow of care/services through the care continuum. Performs discharge planning appropriately towards patient specific goals/outcomes and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care. Our Hospice Nursing Aides may assist patients' family members in caring for their terminally ill relatives by instructing them on how to go about performing basic health-care routines. Nursing Aides consult on a regular basis with their supervising registered nurses in terms of keeping hospice team members apprised of our patients' progression and the families' needs. Qualifications Must meet one of the following requirements: Have a minimum of 1 year of full-time experience in direct client care in an institutional setting (hospital or nursing facility) Have 1 year of experience within the last 5 years in direct client care in a home health or hospice setting Have satisfactorily completed a training and competency evaluation program that complies with federal and state regulations Be registered and listed in good standing on the Nurse Aide Registry in the state of employment Must possess a valid state driver’s license and autombile insurance and have reliable transportation. Must have the ability to recognize the needs of people, to maintain good relationships with people, and have the capacity to make meaningful observations and communicate them to the supervisor. Essential Functions Patient Assessment & Care Planning: Conduct comprehensive assessments and develop individualized care plans based on patient needs and goals of care ensuring optimal comfort and symptom management throughout the patient’s episode of care. Symptom Management & Pain Control: Administer medications, monitor symptoms, and adjust treatment plans to provide optimal relief. Interdisciplinary Collaboration: Work closely with physicians, social workers, chaplains, and other healthcare professionals to coordinate ongoing holistic care. Patient & Family Education: Provide support, resources, and education on disease progression, hospice philosophy, and available support services. Support patients and families in making informed decisions about care and treatment options. Emotional & Psychosocial Support: Provide compassionate care to patients and families, addressing emotional, spiritual, and psychological needs. Documentation & Compliance: Maintain accurate, compliant, and timely records of patient care, interventions, and progress while adhering to hospice regulations. Home & Facility Visits: Travel to patients' homes, nursing facilities, and hospitals to provide direct care.
