• Collaborates with all members of the assigned area’s team to coordinate the care of the patient across the continuum. Is responsible for assisting the clinics and physicians with coordinating and planning of programs, data collection, and quality improvement.

II. TYPICAL PHYSICAL DEMANDS:

• Diploma or Associate’s degree in Nursing

• Bachelor’s degree in Nursing preferred.

• Current certification at BLS/HCP level.

B. EXPERIENCE:

• Two (2) years nursing experience.

• Prior year experience in outpatient setting preferred.

• Demonstrated experience with computer word-processing, spreadsheet, and presentation applications.

• Experience with computer based medical record.

• Knowledge of performance improvement, databases/information systems preferred.
