Manages the daily operation of the Case Management Department staff to provide effective and efficient discharge planning services to patients in accordance with policies, procedures, regulatory requirements consistent with the mission, vision and values of the Jefferson Health Enterprise.Job Duties:
- Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
- Supervises the Department staff at identified hospitals.
- Responsible for the recruitment, hiring and onboarding of all new staff.
- Responsible for updating Departmental policies and procedures, including an annual review of all departmental policies and procedures.
- In conjunction with the Director, collaborates with Hospital Leadership team to establish and implement departmental functions and goals in accordance with both departmental, hospital and enterprise strategic initiatives and goals.
- Actively participates in relevant hospital administrative meetings representing the Department.
- Responsible for completing/participating in completion of annual employee performance evaluations of the staff.
Job Description
Qualifications for this position include:
- MSN Degree preferred, BSN Degree Required OR MSW Required
- RN State Licensure required if BSN/MSN
- ACM Certification highly preferred
- Minimum 3 years Hospital Based Case Management experience required
- Prior management experience required
- Demonstrate effective use of conflict resolution skills as necessary to ensure timely resolution of issues.
- Demonstrate effective prioritization and time management skills.
- Ensures payroll for the staff is completed.
- Responsible for the coaching, counseling and corrective action planning as needed for identified staff.
- Responsible for completing/participating in completion of annual employee performance evaluations of the staff.
- Keeps immediate supervisor, senior leadership and coworkers well informed.
- Audits staff documentation and analyzes metrics to ensure efficient, effective and compliant performance.
- Develops and maintains relationships with post-acute facilities and community agencies to ensure patients have proper access to all available resources.
- Responsible for scheduling staff to maintain proper departmental coverage at each hospital.
- Creates continuing education opportunities for staff.
- Manages the Department activities for the identified hospitals.
- Continues to expand knowledge base of clinical and regulatory requirements related to discharge planning and case management activities.
- Participates in the hiring, orientation and retention of department staff.
- Functions as a resource for staff and other members of the Hospital in addressing discharge planning issues.
- Participates in removing barriers which impede patients’ progression to discharge.
- Actively participates in daily Escalation meetings, focusing on resolving identified patient progression barriers.
- Attend daily Patient Safety/Bed Huddle meetings, representing the department.
- Represents the department in the absence of the Director.
- Actively participates in leadership rounding, meetings and identified projects.
- Performs other duties as assigned.
- Takes “on call” for clinical or administrative responsibilities to address and resolve departmental issues. Escalate unresolved issues to supervisor.
- Case Management Safety Competencies: Identified patients using the appropriate patient identifiers. Utilizes interdisciplinary collaborative team approach to patient care planning and discharge coordination. Ensures patient medical information is properly communicated to external providers/facilities. Communications discharge plan to patient, family and members of multidisciplinary team. Ensures information is transferred to external providers/facilities.
- Knowledge (Comprehension or understanding acquired by experience, study or learning): Understands the impact of developmental stage on adjustment to illness/trauma. Demonstrates ability to identify adolescent behaviors concerning body image, independence, sexual identity, interference with ability to establish identity, tendency to deny or minimize severity of illness. Allows adult patient to maintain control and involves family in decision-making. Continually considers physical, social and emotional barriers and physical limitations while working with the geriatric patient. Demonstrates understanding of end of life issues.
- Skills & Abilities (Application or performance of acquired knowledge): assesses and anticipates discharge planning needs to ensure a safe and effective transition to the next level of care for the patient (home, rehab or skilled facility). Involves family/patient/guardian in the decision-making process related to discharge planning. Exhibits effective communication skills while working with neonate, infant, adolescent, and geriatric patient. Demonstrates ability to recognize the signs of physical and/or emotional abuse or neglect to help identify victims. Follows appropriate policy and procedures for reporting of abuse. Demonstrates the ability to identify hearing and sight deficiencies in geriatric population.
