The UW Medicine Harborview Comprehensive Stroke Center is seeking an experienced, collaborative, and patient-centered Stroke Nurse (RN) Navigator to join our team.

WORK SCHEDULE

Full-time, 100% FTE (40 hours per week)
Monday–Friday (0800-1630)
On-site at Harborview Medical Center

Department Description

The Comprehensive Stroke Center at Harborview Medical Center is a Joint Commission-certified and Washington State-designated Level I Comprehensive Stroke Center, providing high-acuity, evidence-based care to stroke patients across the WWAMI region (Washington, Wyoming, Alaska, Montana, and Idaho).

Our multidisciplinary team is dedicated to improving patient outcomes and access to care through cutting-edge interventions, regional partnerships, telestroke outreach, and robust post-discharge coordination.

Our multidisciplinary team of the stroke center is a nationally and internationally recognized leader in stroke care, innovation, and health equity—delivering coordinated, person-centered services across the care continuum through inpatient, outpatient, and Telestroke partnerships.

The Stroke RN Navigator plays a vital role in patient and family education, complex care coordination, and clinical collaboration throughout the continuum of stroke care—from admission through discharge and outpatient follow-up.

POSITION HIGHLIGHTS

Role

The Stroke RN Navigator plays a critical role within the Comprehensive Stroke Center at Harborview. This role reports directly to the Stroke Program Manager/Director and works in close collaboration with:

Neurologists, neurosurgeons, hospitalists, and stroke fellows/residents
Care management, social work, and therapy services
Outpatient stroke and neurology clinics
Telestroke and WWAMI regional referral partners and local providers

Primary Responsibilities

Patient Education & Family Support

Deliver individualized, in-person stroke education at the bedside
Reinforce understanding of diagnosis, medications, stroke warning signs, and lifestyle modifications
Support family engagement and caregiver readiness throughout the hospitalization

Inpatient Navigation & Care Coordination

Identify patients through daily census and discharge reports
Conduct chart reviews for patients admitted within the last 12–72 hours
Attend and participate in interdisciplinary discharge rounds
Collaborate in person with medical teams, care management, and therapy to facilitate safe, timely discharges
Coordinate services with the Comprehensive Care Nurses (CCN), Care Assurance Team (CAT), and external partners as needed

Post-Discharge Follow-Up & Continuity of Care

Perform outreach calls to discharged patients to ensure successful transitions
Coordinate follow-up appointments with primary care and specialty providers
Facilitate post-discharge imaging, lab work, or therapy services at HMC or local facilities
Assist with outpatient stroke clinic referrals and care handoffs

Program Support & Quality Improvement

Attend and participate in stroke operations and performance improvement efforts as directed
Support chart reviews, data collection, and quality audits
Support and participate in monthly Stroke Support Group, UW Stroke Club, for survivors and families
Help identify gaps in care, patient safety concerns, or workflow barriers and escalate to stroke leadership

Required Qualifications

Bachelor of Science in Nursing (BSN)
Current license to practice as a Registered Nurse (RN) in the State of Washington
Minimum 3 years of RN experience, ideally in neurology, neurosurgery, critical care, acute care, or care coordination working closely with stroke neuro patient population
Strong communication skills, both verbal and written, and ability to engage with multidisciplinary providers without constant direct supervision
Comfort with Epic EHR system, MS Office software, and other care coordination tools

Job Description $10,000 Sign-On Bonus to eligible new employees joining UW Medicine UW Medical Center-Northwest - Vascular Center has an outstanding opportunity for an Assistant Nurse Manager (RN3) . This is a supervisory position reporting to the Clinic Manager. WORK SCHEDULE Full time / 40 hours per week Monday-Friday from 7:00am-3:30pm Department Description Our Vascular Center is a fast-paced, multifaceted clinic providing a broad mix of inpatient and outpatient procedures, each enhanced by the latest diagnostic and therapeutic technologies. Our vascular surgeons take a multidisciplinary approach to care and are uniquely equipped to tackle challenging diseases, acute and chronic alike. Common procedures we perform include carotid endarterectomy, TEVAR/EVAR (endovascular thoracic/aortic repair), varicose vein stripping, first rib resection, carotid and iliac artery stenting, saphenous vein ablation and fistulograms with angioplasty. POSITION HIGHLIGHTS Be part of an innovative, state-of-the-art clinic Supervise a close-knit team Work closely with the Clinic Manager to facilitate daily operations of the Vascular Clinic and Vascular Procedure Center Primary Job Responsibilities Establishes and maintains credibility as the leader of day-to-day clinic operations Identifies staff and resource problems and takes necessary steps towards a resolution Provides feedback to the Clinic Manager for annual performance evaluations Responsible for staff schedule and Kronos timekeeping in collaboration with Clinic Manager Resolves unplanned absences, following Ambulatory Services Resource Pool Protocols Develops, coordinates, and maintains work processes to ensure the clinic runs successfully Establishes and maintains safety and emergency protocols for the clinic Assists with screening / interview process for Registered Nurses and Medical Assistants Manages orientation and educational programs for newly-hired staff Performs Annual Competencies for RN & MA staff In-basket triage, telephone triage, care coordination Promotes effective communication among staff and patients Performs general clinic RN responsibilities when needed Requirements 2 years of recent ICU, PACU, vascular, procedural or ambulatory nursing experience Familiarity with conscious sedation Previous charge, lead, or supervisory work experience BLS (required by start date) ACLS (required by start date) Active licensure to practice as a Registered Nurse in Washington State About Uw Medical Center-northwest UW Medical Center is an acute care hospital located in Seattle with two campuses: Montlake and Northwest.
