POSITION OVERVIEW: The RN Case Manager is a direct link between medical services and the ICF/IRA residents and staff as well as with clinical professionals outside the program. The RN Case Manager is responsible for the complete nursing care and performance of all routine medical procedures within the scope of the nursing practice. The RN Case Manager is responsible for supervising LPN’s, AMAP and Direct Support Professionals (milieu counselors) in the performance of nursing tasks and activities. The RN must possess a working knowledge of OPWDD rules and regulations and JBFCS policies and procedures. The RN must be capable of supervising, understanding and working effectively with diverse populations. KEY ESSENTIAL FUNCTIONS: Provide direct nursing care as prescribed by the physician/Nurse Practitioner, in compliance with recognized nursing standards and State and Federal regulations. Uses nursing process to make continuing assessments of the individuals with developmental disabilities total health needs. Maintain direct interaction with consumers on a regular basis, but in no case, at a frequency of less than once per week. Perform nursing assessments, by direct physical examination, of each consumer upon entry into the program quarterly (at a minimum) thereafter. Develop and implement an individualized plan for nursing service. The nursing care plan must be updated at least annually or whenever there is significant change in the consumer’s condition. Coordinate all acute and ongoing health services, as indicated by need and regulatory mandate. Monitor health and behavioral status, labs and efficacy of prescribed treatments. Ensure timely communication of findings to appropriate clinical personnel. Ensure that all medical record documentation is accurate, timely and outcome based and reflected in the nursing notes. Provide initial and ongoing, but at a minimum frequency of annually, training to staff in all nursing tasks, functions, protective and preventive health measures that include but are not limited to: Appropriate health and hygiene methodologies Control of communicable diseases and infections Detection of signs and symptoms of illness, first aid for accidents or illness and basic skills required to meet the health needs of the consumers Client specific medical/health related issues Seizure management Ensure that training in the above noted areas is given to all new hires and per diems prior to their assuming client specific work assignments. Provide training and ongoing support to direct care staff regarding medication administration, tube feeding and diabetic care utilizing the standard curriculum approved by OPWDD. Conduct annual clinical performance evaluations for unlicensed direct support staff for procedures that include but are not limited to: medication administration, tube feeding and insulin administration. Evaluations must become part of the employee’s annual performance evaluation. Maintain documentation of all training curriculums, attendance and post training testing. Must have experience and ability in working with electronic health record management, electronic care coordination and electronic health record interface with external service providers. Active participant in the Interdisciplinary Team (IDT). All other duties, as assigned. POSITION OVERVIEW: As a member of the ACT team, the RN is responsible for providing a range of treatment to clients with a SPMI/SUD, rehabilitation and support services within the scope of the nursing practice for the clients receiving services from our ACT teams across the city. S/he is responsible for conducting initial nursing assessments and ongoing health monitoring assessments; assessing physical health needs; making appropriate referrals to community physicians as needed, providing medical case management and administration of medication in conjunction with the prescriptions/orders of psychiatrist, PNP, and medical providers. Conduct weekly visits thereafter for clients currently residing in homeless shelters and assigned to the ACT program and/or in client’s homes or elsewhere in the community where the client is accessible. Develop a treatment plan with the team, based on clients’ needs. Ensure that any medical diagnoses are addressed and the client will be educated as to conditions and signs and symptoms. Complete nursing assessments and health monitoring assessments within appropriate timeframes. Will follow/implement all direction from Psychiatrist or PNP regarding medical treatments and medication orders. Will administer prescribed and/or routine medical treatments, support clients and staff with self-administration of medication procedures and evaluate for potential side effects or adverse reactions. Ensure that all internal policies regarding such administration are adhered to. RN will partner with our clients, in a respectful, encouraging way to promote healthy lifestyle changes within a harm reduction framework, with shared decision-making practices. Will coordinate and deliver medications to the client’s residence or shelter, be responsible for medication counts, follow-up the pharmacies (i.e. refills, insurance authorizations etc) and administering medication to clients. Make referrals, schedule appointments and accompany the client when required. Make visits to hospitalized clients to coordinate care. Provide advocacy for hospitalized clients. Assist with follow up care to promote complete recovery. Function as a liaison between external medical health care providers, shelters, other community organizations and the ACT team with regards to the client’s medical needs. Responsible for health education and communicating information regarding good health practices to the clients under RN's care. Train staff in these practices as well so as to provide continuous health support to clients. Facilitate groups. Respond to medical emergencies as appropriate. Perform all necessary charting and other documentation as required by internal practice and external regulatory bodies within a timely manner. Follow-up and monitor lab results for any indications of change for medication and toxicology; communicate such change(s) to the psychiatrist and PNP.
