Assess resident needs, develop care plans, administer nursing care, evaluate nursing care, and supervise CNAs and other personnel in the delivery of nursing care. Responsibilities of the Registered Nurse (RN) Care Manager: Identify and participate in process improvement initiatives that improve the customer experience, enhance workflow, and/or improve the work environment. Complete assessments of residents at admission and discharge. Supervise nursing staff assigned and allocate duties to LPNs and nursing assistants, as directed. Provide leadership and direction to the Certified Nursing Assistants and Nurses assigned to the unit. Ensure that routine nursing care is provided to meet the daily nursing care needs of each resident. Make daily rounds to monitor resident care and status of residents. Implement established nursing policies and procedures, educating nursing support staff according to facility guidelines. Review charting on a weekly basis for accuracy and completeness. Maintain a safe working environment, complying with infection control and body mechanic procedures. Respond to need for staffing shortages by arranging for replacement staff. Responsibilities of Certified Nursing Assistant (CNA): Provide personal care (i.e., grooming, bathing, dressing, oral care, etc.) of residents daily and as needed. Change bed linen and maintain appropriate grooming supplies in resident room on a regular basis. Ambulate and transfer residents, utilizing appropriate assistive devices and body mechanics. Record resident vital signs, weights and measurements, intake and output, food consumption, and other information, as assigned. Routinely turn and position residents, as necessary. Observe residents carefully and report changes in condition to Charge Nurse. Participate in collecting data needed for the accurate completion of the MDS and the plan of care. Provide input to the nurses for development of the plan of care. Provide adequate hydration to residents at regular intervals each day according to specific resident needs. Prepare residents for each meal and transport to dining room (unless a bed patient) making sure that food tray is accessible, self-help devices are available (if needed), and assistance is provided to those residents who cannot feed themselves. Keep an accurate record of food intake and output, as directed. Be alert for proper ventilation, temperature, light and noise control. Take TPR (temperature, pulse, & rate), blood pressure, weighing of residents, etc., as directed. Collaborating with other healthcare providers to investigate and control outbreaks of infectious diseases within the facility. Providing guidance and support to staff members on infection control issues and serving as a resource for questions and concerns. Participating in quality improvement initiatives related to infection control and patient safety. Keeping up-to-date on the latest developments in infection control practices and incorporating new guidelines and recommendations into facility protocols. Monitoring and assessing the facility for potential infection risks and implementing interventions to prevent the spread of infection. Educating staff on proper infection control practices, including hand hygiene, personal protective equipment use, and cleaning and disinfection protocols. Conducting regular audits and inspections of the facility to ensure compliance with infection control standards. Requirements of the Registered Nurse (RN) Staff Educator: Current Registered Nurse (RN) license in NY. Must have 2 years of experience as an RN Staff Educator in a skilled nursing facility.
