Here, you will work alongside compassionate physicians, nurses and other healthcare staff to offer patients support and comfort as they adjust to the hospice environment. Hospice helps patients and their families prepare physically, emotionally and spiritually for the end of life. Our focus is on providing comfort, not a cure, and on quality of life, not duration. As an RN this team, you will collaborate with physicians and fellow caregivers to support patients within our Care Center. In this role, you can make a difference in how they view and adapt to an uncertain future, no matter what that may hold. Additionally, this position offers a flexible schedule and autonomy to work independently, allowing you to maintain a healthy work-life balance while continuing to serve as an emotional ally and comfort. This is a PRN position. A caregiver in this role will work the first 4 weeks, 2 days from 8:00am to 4:30pm, and 2 days from 7:00am to 7:30pm. After that, caregivers will work 3 12-hr shifts until the 12th week. A caregiver who excels in this role will: Provide supervision, assessment, planning and direct patient care to hospice patients. Perform accurate and complete admissions assessments and re-assessments. Assess patients for safety risks, abuse and neglect. Assess, document and manage patient’s pain. Prioritize and document patient problems. Administer medications and treatments safely and accurately with a focus on patient comfort. Coordinate care for managing a caseload of patients. Document changes in patient condition and effectiveness and timeliness of care/interventions. Evaluate, update and revise plans of care. Assess educational needs of patients, caregivers and family while considering cultural issues and other special challenges. Provide teaching and educational material to patients and families concerning diagnosis, diagnostic tests and procedures, safe and proper use of equipment, reason for and safe use of medication food/drug interactions, injury prevention, self-care and nutritional needs.
