Utilize assessment skills in providing patient care in all phases of cardiac rehabilitation. Educate patients/families about cardiopulmonary diseases and treatments. Participate in case management and discharge planning conferences with other interdisciplinary staff. Document patient progress and collaborate with various members of the healthcare team to meet patient needs. Develop rehabilitative goals to prevent progression of heart and lung disease (exercise prescription, activities of daily living, risk factor reduction, prevention of exacerbations, etc.).
