Job description Cordial Health is seeking a dedicated and compassionate Psychiatry Nurse Practitioner (PMHNP) to join our team. We are a leading healthcare organization providing comprehensive mental health services to patients in nursing homes across Genesee County , NY. Responsibilities: Conduct comprehensive psychiatric evaluations and assessments of elderly patients in nursing homes. Diagnose and develop individualized treatment plans based on the patient's mental health needs. Provide medication management and adjust treatment plans as necessary. Collaborate with interdisciplinary teams, including physicians, psychologists, and social workers, to ensure holistic patient care. Offer counseling and psychotherapy services to patients and their families. Educate patients and their families about mental health conditions, treatment options, and self-care strategies. Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards. Participate in clinical meetings, case conferences, and quality improvement initiatives. Requirements: Valid Nurse Practitioner license in the state of New York with specialization in Psychiatry (PMHNP). Ability to work independently and effectively manage a caseload of patients. Strong diagnostic skills and knowledge of psychiatric medications and treatment modalities. Excellent interpersonal and communication skills to establish rapport with patients, families, and interdisciplinary team members. SUMMARY As a Nurse Practitioner, your clinical skills and superior customer service in providing medical care leads to a positive, effective patient experience. The NP in this role will float to different WorkReady locations. STATUS: Part Time (24 hours) LOCATION: WorkReady Locations DEPARTMENT: Employee and Occupational Health SCHEDULE: Days, Monday-Friday ATTRIBUTES Excellent customer service and leadership skills Patient focused, with demonstrated care and compassion Exceptional communication and documentation skills Critical thinking skills, decisive judgment and the ability to work with minimal supervision RESPONSIBILITIES Patient Care. Synthesize and interpret history, physical examination and diagnostic testing data; develop a diagnosis, initiate treatment and health maintenance plan; select and order appropriate diagnostic and health maintenance procedures; refer patients to collaborating physician, specialist and/or appropriate community services or agencies; prescribe medication, perform minor procedures and coordinate treatment Leadership. Provide health education, instruction, guidance and advisement to patients regarding medication and treatment; counsel family members and recommend community resources to meet patient and family needs Administrative Duties. Perform administrative functions to include completing insurance and when time permits, medical discharge documentation, providing guidance and teaching to ancillary personnel, communicating with and counseling family members, preparing daily billing sheets. PAY RANGE: $100,000.00 - $150,000.00 CITY: Rochester POSTAL CODE: 14622 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
