Job Summary : The Advanced Practice Provider Trainer is a member of the clinical practice team and works collaboratively with the fellow team members in providing patient care. This role is responsible for training and evaluating 2 – 3 new, hired in the last two years, Advanced Practice Providers, nurse practitioners and/or physician who are not participating in the NP Residency program. As part of the team-based approach to care, the Trainer participates in coordinating care for individual patients, supports patients and families in self-management, through evidenced-based approaches as may be appropriate, engages in health center projects that are aimed at specific patient population needs, and utilizes effective and appropriate communication strategies, such as health literacy approaches when working with patients. Essential Functions: Mentoring/Training: (approximately 20 hrs.) 1. Supervise, train and evaluate new Advanced Practice Providers (NP and/or PA). 2. Perform chart reviews based on an established schedule. a. All charts for the first month of a new hire’s employment. b. Up to 10 chart reviews for each month following. 3. Coordinate MAVEN Services which include: a. Coordinate CME educational sessions with site medical directors b. Meet with APP assigned MAVEN mentor at designated times to begin after new APPs 3rd month of employment 4. Observe and shadow APP 5. Evaluate APP based on accepted competencies which will include: a. History taking b. Physical exam skills c. Analyzing data d. Management/ plan formation e. Procedures f. Problem solving hazards and biases g. Medical practice and principles 6. Provide constructive feedback to APP for the course of two years and possibly longer as the APP develops into the role. 7. Support organizational compliance and training requirements and work with APPs to complete any required or newly required trainings. 8. Participate in Administrative Projects as assigned by the Medical Director. Clinical: (approximately 20 hrs.) 1. Provide direct medical care for four sessions per week , complete clinical administrative work four hours per week. 2. Adhere to established protocols, responds to medical emergencies within the facility assigned. 3. Assists/instructs other health care employees in the management of client care. 4. Make referrals to health care professionals, specialists, other clinics and community agencies. Experience and Skills: .Current and unrestricted APP (Nurse Practitioner or Physician Assistant) license in Massachusetts · Board Certified in Family Medicine preferred, (Board Certified with ABMS) · Knowledge of Quality Improvement Principles and Practice · Knowledge of clinical problems and barriers to access of vulnerable patient populations · Minimum 3 years’ experience working with Electronic Health Records (EHR). · Excellent communication and problem-solving skills. · Management experience within an ambulatory setting. Summary The Assistant Medical Director will work both as a Primary Care Provider and also as a Clinical Leader at our Worcester Primary Care site. This person will be responsible for managing, evaluating, and supporting the clinic’s Advanced Practice Clinicians (NPs and PAs). This Assistant Director will also support the APP Residency by participating in precepting Residency sessions, meeting with Residency Director, and acting as liaison between Residency and other administrative teams within the organization. The Assistant Medical Director will provide comprehensive primary care as a member of the clinical practice team, working collaboratively with other team members in providing services to patients. This includes participating in the coordination of care for individual patients, and supporting patients and families in self-management, through evidenced-based approaches as may be appropriate at the clinic. Essential Duties & Responsibilities Clinical : 1 Provide direct medical care to a panel of patients for five sessions per week ( 20 hours). Complete clinical administrative work 5 hours per week. 2. Adhere to established protocols, responds to medical emergencies within the facility assigned. 3. Assists/instructs other health care employees in the management of client care. 4. Make referrals to health care professionals, specialists, other clinics and community agencies. Administrative : 1. Work on grant funded responsibilities as needed. 2. Work on managerial responsibilities 15 hours per week (see below responsibilities) 3. Review and sign off on bi-weekly timesheets of Advanced Practitioners at the Worcester primary care site. Work with the medical director and scheduling coordinator to determine if time off requests can be approved based on patient needs. The Assistant Medical Director will arrange and manage schedules of students and medical residents who precept and shadow at primary care site. Complete, with the assistance as needed of the Medical Director and HR, APP staff’s performance evaluations (as applicable), on rolling anniversary date schedule The Assistant Medical Director will work with the Medical Director and HR to interview, hire, and onboard new providers to Tacoma site. Collaborate as a member of the operational team in Worcester which consists of Medical Director, Clinical Practice Manager, Nurse Managers, as well as other leadership team members. Participate in Quality Assurance and Quality Improvement Activities as assigned by the Medical Director. Participate in Administrative Projects as assigned by the Medical Director. Provide Clinical Supervision and mentoring to Nurse Practitioner, Medical, and Physician Assistant students by mutual agreement with the Medical Director. Assist in improvements in the EHR by being a member of the EMR team-which includes operational improvements, quality requirements, and being a champion/super user. Perform other duties and work assignments as assigned by the supervisor including attendance of requested department, center, and outside meetings and/or trainings. Qualifications: Master’s Degree as a Physician Assistant or Nurse Practitioner Active, Unrestricted License in Massachusetts, DEA and MCSR. Minimum of three years of experience in a primary care setting. Salary range is $125,000-$135,000 depending on years of experience Preferred Qualifications: 1. One to two years leadership/administration experience 2. Bilingual in English/Spanish or English/Portuguese.

Summary We are looking for applicants with 3+ years of experience. The Urgent Care APP is a member of the clinical practice team and works collaboratively with other staff members to provide high quality patient care according to the PCMH model. Urgent Care Medicine APPs at Kennedy Community Health will have the opportunity to work in partnership with the numerous other disciplines offered at the health center including mental and behavioral health, pharmacy, Medication Assisted Treatment (MAT) program, Reproductive Health, Gender Affirming Care, Dental and Optometry. Clinical Responsibilities : Provides medical care to patients with acute medical problems or acute exacerbations of chronic illnesses. Examines patients, formulates diagnostic plans, and orders diagnostic testing. Interprets examination findings and test results and implements treatment plans. Provides acute care for patients across the entire lifespan, including infants and pregnant patients. Determines the need for consultation and assists in medical care and treatment provided by other specialists. Participates in other care procedures according to training and demonstrated ability. Prescribes and assesses effectiveness of pharmaceuticals, other medications and treatment regimens as appropriate for patient’s medical conditions. Promotes continuity for patients with the primary care team within EMK and facilitates appropriate follow up with primary care team when appropriate Meets the Health Center’s annual productivity goal. Maintains and updates Electronic Health Records (EHR) in a systematic format and completes required documentation within 48 hours. Our schedule includes weekday evening hours and Saturday morning hours on a rotating basis. Required Qualifications Completed Master’s level Nursing Practice Degree and Family Nurse Practitioner certification OR Completed Master's Level Physician Assistant Degree PA Board Certification OR FNP Board Certification Active, Unrestricted FNP or PA License in Massachusetts.
