Specialty certification, advanced procedures with appropriate privileging, weekend and on-call coverage on a rotational basis, as required. E-consults and telehealth could be included in the care provided. Essential Responsibilities Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, discharge planning and providing accurate medical record documentation for patients in physician office(s) and non- acute based settings (20%) Orders, interprets, and evaluates laboratory, radiological, and diagnostic studies appropriate for the patient's condition to determine and implement treatment plans. (10%) Makes medical diagnoses and institute appropriate therapy within the applicable scope and standards of practice. (10%) Institutes emergency measures, treatment, or appropriate stabilization in acute patient situations such as cardiac arrest, hemorrhage, or shock. (10%) Ensures appropriate understanding and timely documentation in the EMR to support accurate medical coding and billing, and record/report pertinent information to the attending physician of record. (10%) Educates patients and family members regarding health and illness prevention, medication and treatment instructions, and recommend community resources as needed. (10%) Communicates effectively with the attending physician regarding the plan of care and involve them in clinical decision-making for any issues outside the Physician Assistant's knowledge base or scope of practice. (10%) Maintains mandatory professional continuing education, actively participate in patient safety, quality improvement, and patient engagement initiatives, and maintain professional practice consistent with organizational and advanced practice mission statements and specialty-specific privileges. Qualifications Applicant must hold a Master’s degree from an accredited NP program. Applicant must be a certified NP (ACNP). Current State of Ohio licensure is required and should be maintained at all times. Certificate to Prescribe must be current and maintained at all times. Applicants must have a minimum of five years of experience as a Registered Nurse in an acute care facility; 1 year experience as a NP/ACNP preferred; and experience as a critical care nurse is preferred. Evidence of malpractice insurance must be provided as part of the application process. A thorough understanding of HIPAA law is required and professional discretion must be demonstrated at all times. Applicant must have excellent oral and written communication skills. Excellent communication skills with both patients and colleagues are essential. Computer literacy is required and must include Excel, Word and Outlook, and Electronic Medical Records experience is an advantage, preferably EPIC.

This individual will review Radiology reports, talk with the patients, order additional studies, and communicate the results of those studies to the appropriate providers to provide additional care. They will be viewed as partners with the primary care physicians and the population health team, identifying ways to ensure that our patients receive the appropriate follow-up care for their health.
