The LPN is responsible for assessing patient health problems and needs, implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration required. ESSENTIAL DUTIES AND RESPONSIBILITIES The essential functions include, but are not limited to the following: Provide skilled nursing care under the direct supervision of the Clinical Supervisor Complete evaluation tasks, including vital signs and medication reviews Coordinate with physicians, therapists and other individuals in the client’s care plan Assess and chart observations of the client’s condition at each visit Maintain complete and accurate documentation of nursing care Work collaboratively with all members of the health care team Provide education to clients and families on proper home health care procedures and strategies Record patients' medical information and vital signs. Monitor, record, and report symptoms or changes in patients' conditions.
