Completes forms and documents and obtains physician signature as appropriate. This includes but not limited to diabetic supply form, DME, and Home Health Orders. 5. Arranges referrals for specialty care, urgent or emergent care at the direction of the provider. Instructs patients in home self-care when an immediate clinic evaluation is not necessary. Instruct patients to come to the clinic as necessary. Consult with the clinic manager or provider when there is a question about the severity of the problem or the need for immediate attention. 6. Administers oral and parenteral (IM, subcutaneous, intradermal) medications as ordered and prescribed by the provider using the 6 Rights of Medication Administration. Documents all nursing care and treatments performed, in the electronic medical record. 9. Performs hospital follow up by reviewing discharge summary and performing med reconciliation. 11. Follow up on any referrals. 12. Delegates to MA within scope of practice to enhance patient care. 13.
