Your Job Summary The Certified Medication Technician (CMT) will be responsible for administering daily medication to residents in accordance with accepted standards of practice, state and federal regulations and licensing requirements. Duties include monitoring residents, reporting changes, and collecting and recording medication administration. Principal Responsibilities • Administers prescribed medications and treatments as defined by state regulations in accordance with approved techniques and notes time and amount on resident’s chart. • Deliver routine daily medications as prescribe by physician to residents; pass resident medications in accordance with physician orders and as stated on the Medication Administration Record (MAR). • Verifies identity of resident receiving medication and records name of drug, dosage, and time of administration on specified forms and records; Documents reasons prescribed drugs are not administered and reports to nursing supervisor. • Presents medication to residents and observes ingestion or other application, or administers medication, using specified procedures • Document completion of tasks delivered on a daily basis; Document any observed change of condition in accordance with local rules and regulations. • Communicate between Med Techs/Med Aides about resident needs for consistent quality services. • Participate in meal service including serving, clearing, and cleaning the dining room; Assures that residents are positioned appropriately for meals; Assures that appropriate utensils are within reach of residents; Serves food trays and assists with feeding as indicated; Delivers and documents all a.m., p.m., and bedtime snacks and supplements; Provides hydration to all residents according to center policy. • Responsible for knowledge of proper procedures to provide personal care services to residents; provide requested personal care services to residents. • Observes resident, records significant conditions and reactions, and notifies Nurse Supervisor or Physician of resident’s condition and reaction to drugs, treatments, and significant incidents. • Records and restocks medication inventories. • Prepares equipment and aids nurses during treatments and examination of residents. • Checks residents in section regularly; Answers call light promptly; Follows through with resident problems or needs by delivering care or reporting to supervisor depending upon level of ability and nature of resident need. • Provides residents’ personal hygiene by giving bedpans, urinals, baths, back rubs, shampoos, and shaves; assisting with travel to the bathroom; helping with showers and baths. • Provides resident comfort by utilizing resources and materials; transporting residents; answering resident’s call lights and request; reporting observation of the resident to nursing supervisor. • Other duties, responsibilities and activities may change or assigned at any time with or without notice. Qualifications • Certified in the state of practice, required. • Successful completion of State Approved Nurse Aide Training and Competency Evaluation Program; Name must appear on State Registry as Nurse Aide in good standing. • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. • Current BLS/CPR certification (All SNFs; ALFs as required by state). POSITION DESCRIPTION: A CMA is under the direct supervision of the charge nurse. The charge nurse will follow up with the CMA to ensure the med pass is completed timely and accurately to assure that the highest degree of quality resident care can be maintained at all times. QUALIFICATIONS : Ability to communicate effectively in English through both speech and writing. Previous patient care experience preferred. Current certification for Medication Aide. RESPONSIBLITIES Reports promptly any change of patient condition or other concerns to Patient Care Supervisor. Follow charting guidelines for PRN documentation. Reports medication reorder. Participates in all assigned meetings, trainings, education and in-service as required. Inventory documentation of drugs on appropriate forms. Current certification for Medication Aide.
POSITION DESCRIPTION Responsible for building referral source relationships in a variety of settings, such as hospitals, clinics, and senior-service related organization within the community. To build and grow census and quality mix by developing the market, working with acute care and other referral sources and providing prospective residents and/or responsible parties with appropriate information and assistance to choosing the nursing center QUALIFICATIONS • Graduate of an approved Licensed Practical/Vocational Nurse program and licensed in the state of practice, required. • One-year experience providing specialized treatments in long term care setting. • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. • Current BLS/CPR certification (All SNFs; ALFs as required by state) RESPONSIBILITIES . • Perform resident skin assessments, obtain treatment orders, administer treatment as ordered, and document appropriately to ensure that resident skin integrity is improved and/or maintained. • Documents findings appropriately and contacts physician to make recommendations for new or revised treatment orders; Monitors the progress of wound healing and makes recommendations for changes as appropriate. • Monitors the progress of wound healing and makes recommendations for changes as appropriate. • Maintain records of pressure ulcers and skin alterations including risk assessment, prevention and minimizing activities; Performs and records weekly pressure ulcer assessment and measurement documentation; Identify and maintain effective methods to gather pressure ulcer and skin alteration related information; Calculates weekly / monthly pressure ulcer rates. • Investigate and identify center practices and risk minimizing measures, including accurate completion of Braden risk scales on admission, implementation of preventive interventions. • Monitor timely implementation of preventive interventions for at risk residents, including new admissions, individuals with skin alterations and individuals at risk for development of skin alterations. • Educate newly hired nursing staff on Skin Practice Guide requirements and Skin Resource Library including wound assessments, preventive interventions, care planning, treatment selection options, documentation requirements and education resources available. • Evaluate availability of needed supplies for wound care and coordinate ordering with central supply. • Assist the Director of Nursing and medical director with case review for center acquired pressure ulcers. • Monitor changes in state and federal regulations and current industry-standard practices; Keeps current with industry trends through ongoing specialized education in the area of skin and wound management • Provide monthly, quarterly and annual reports including recommendations for changes in center practice for the Quality Assurance and Performance Improvement Committee. • Other duties, responsibilities and activities may change or assigned at any time with or without notice.
