Responsibilities •Complete and scan required record keeping forms/charts upon the guest’s admission, transfer, and/or discharge.•Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the guest, as well as the guest’s response to the care.•Report all discrepancies noted concerning physician’s orders, diet change, charting error, etc., to the Director of Nursing.•Ensure that appropriate documentation concerning unauthorized discharges is entered in the guest’s medical record in accordance with established procedures. Qualifications •Current, unencumbered LPN/LVN license required or Compact State license- required.•A minimum of 2 years LPN/LVN experience required.•Able to educate patients and family on multiple disease processes and have good medication knowledge and wound care experience. Job Duties: Administers medication and performs treatments on an assigned unit as ordered by the physician and within scope of license Medications may be in a variety of forms including oral, enteral, parenteral, suppository, etc. Treatments may include sterile and aseptic wound cleaning and dressing, postural drainage and percussion, tracheostomy, gastrostomy and jejunostomy care and catheter care Observes and reports the effectiveness of medications and treatments Ensures an adequate amount of supplies and medication are available Receives and delivers a variety of information regarding the residents’ medical, social, functional and other needs Participates in the receiving and giving of shift reports Communicates with residents, family and staff as necessary to ensure the resident receives appropriate care from the interdisciplinary team Completes the initial admission nursing assessment tool on a newly admitted resident. Assists with the development of the Minimum Data Set (MDS) assessments, ongoing quarterly reviews and care plans Supervises subordinate staff and assists as need with direct resident care for the completion of Activities of Daily Living (ADL’s) including bathing, dressing, observations, treatment and summary of resident progress Conducts and participates in in-service training and staff development programs for regularly employed staff and other duties as assigned Attends and participates in staff meetings May collect and analyze data for quality improvement and infection control programs.
