Primary Responsibilities: Coordinates scheduling of home health patients with the assistance of the supervisor. Responsible for maintaining the team master schedule and visit verification. Complete other assigned duties consistent with the nature of this work. Qualifications: Minimum of three years’ previous work experience, with a preference for home health experience. LPN Licensure in state of practice strongly preferred. Demonstrate a commitment to service to others and a willingness to help others in a team-based environment. Knowledge of appropriate communication skills to include the ability to communicate with both internal and external customers and effectively present information one-on-one or in group settings.
