Works under the direction of the Clinical Manager to rearrange schedules in situations of call-offs, conflict, or high utilization of service. Ensures prompt scheduling of emergent / as-needed visits as directed by the Clinical Manager. Assures timely completion of individual staff schedules within established time parameters. Accesses patient and provider schedules, authorizations, and physician orders to determine appropriate visit schedules under the direction of the Clinical Manager. Assures efficient use of staff by utilizing full-time staff prior to scheduling part-time and per diem staff. Coordinate, assign, and notify appropriate on-call staff and management of the on-call schedule. Work alongside RN Case Managers and others to ensure that patient needs are addressed. This includes ordering supplies, communicating with patients, following up on patient requests, etc. Work with the intake team and branch operations to make contact with new patients. Complete other assigned duties consistent with the nature of this work. Qualifications: Minimum of three years’ previous work experience, with a preference for home health experience. LPN Licensure in state of practice strongly preferred. Demonstrate a commitment to service to others and a willingness to help others in a team-based environment. Knowledge of appropriate communication skills to include the ability to communicate with both internal and external customers and effectively present information one-on-one or in group settings. Ability to solve problems involving many variables in both standard and exceptional situations. Proficient in Excel and Word. Familiarity with other Office programs is a plus. Must have excellent verbal and written communication skills. Perform patient visits if necessary. Qualifications Registered clinician with two (2) years in community health/home health program. Previous supervisory experience is a plus. Bachelor’s degree in a related field preferred. License and current registration to practice in a related discipline in Illinois and Missouri or willingness to acquire licensure in Illinois. In-depth understanding of Medicare Conditions of Participation and payment mechanisms for home health. Strong organizational, planning, problem-solving, decision-making, and leadership skills required. Strong interpersonal skills. Ability to communicate effectively in writing and verbally with people who have varied backgrounds. Ability to direct and manage a multi-disciplinary team of home health care clinicians, both professional and para-professional. Computer skills and ability to use word processing, databases, spreadsheets, applications, etc. preferred.
