ESSENTIAL JOB FUNCTIONS: • Utilizes the nursing process by demonstrating appropriate assessment, planning, implementation and evaluation of residents wound and skin integrity • Collecting and documentation of residents’ skin status including medical history. • Wound care prevention and prevention through plan of care • Sets priorities for nursing action for wound care and prevention • Communicates with physicians regarding resident wound needs • Initiates nutritional interventions to ensure that residents with wounds are provided nutrition to promote wound healing. • Performs direct resident care utilizing established procedure, policies, and standards • Administer treatments and procedures in accordance with the physician’s orders • Maintains the comfort, privacy, and dignity of each residents while performing treatments • Communicates and interacts effectively, courteously, and tactfully with residents, visitors, family members, peers, and supervisors • Conducts rounds frequently to ensure that residents with incontinence are receiving pericare and preventative ointments timely. Discipline non-compliant staff accordingly with consultation from the DON • Performs duties in a safe manner in accordance with established policies and procedures • Performs all job responsibilities in accordance with prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves and proper disposal of soiled material EDUCATION/EXPERIENCE: 1. Review daily the documentation of the dispensing of the controlled substances and narcotics. Ensure that drugs covered by controlled substances laws are verified by inventory. 7. Place pharmacy orders, for and administer all newly prescribed medications and document. 8. Carry out direct contemporaneous charting in your shift. Chart detailed monthly evaluations of each Guest that reflect his/her condition and progress. 9. Responsible for all nursing care of assigned Guests while on duty. Must notify appropriate persons if there is any significant change in a Guest’s condition or any transfer to a hospital. 10. Ensure that Guest care plans are being followed and asses each Guest’s status in accord with their care plan. 11. Round with physicians when needed. 12. Administer or supervise all treatments prescribed by physicians including but not limited to pressure ulcer care, Foley catheter care, and hot and cold compounds. 13. Supervise serving of prescribed diets and fluids intake and immediately report any issues to the physician and DON/ADON. 14. Must be knowledgeable of individual care plans and support the care planning process by reporting specific information and observations of the Guest’s needs, preferences and report any behavioral changes. 15. Provide wound care when needed. 16. Completes medical records documenting care provided and other information in accordance with nursing policies while maintaining strict confidentiality. 17. Assure that established infection control and standard precaution practices are maintained when providing care. Follow established safety precautions when preforming tasks and using equipment and supplies. 18. Reports all hazardous conditions, damaged equipment and supply issues to appropriate persons. 19. Staff Nurse (Registered Nurse/License Practical Nurse) JOB SUMMARY Responsible to provide direct nursing care to the customer, and to supervise the day-to- day nursing activities performed by the nursing assistants. Report all concerns, complaints made by the customer or their family to the Administrator. Counsel/discipline nursing personnel as requested or as necessary. Make daily rounds to ensure nursing personnel are performing required duties, and to ensure appropriate procedures are being followed. Make physical rounds to all customers daily. Make rounds with physicians as necessary. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department. Attend and participate in continuing education programs. Recommend to the Director of Nursing the equipment and supply needs of the department. Place orders for medications and treatments as necessary following established budgetary guidelines. Utilize PCC 24-hour summary concerning recommendations/concerns related to customer care needs. Fill out complete accident/incident reports. Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the customer, as well as the customer’s response to the care. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Ensure the nurses’ notes and monthly summaries reflect the care plan and is being followed. Attend staff meetings, etc., as directed. Perform other related duties as assigned.
